Description
Overview of the medicine
Lidocef 500 mg Capsule contains Cefadroxil, which is a first-generation cephalosporin antibiotic. It is used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
General dosage: 500 mg to 1 g once or twice daily, depending on the type and severity of infection. For most infections, 1 g once daily or 500 mg twice daily. For severe infections, up to 1 g twice daily.
Elderly
No specific dosage adjustment is generally required in the elderly unless there is significant renal impairment. Renal function should be monitored.
Renal_impairment
Dosage adjustment is necessary. For creatinine clearance 25-50 mL/min, 500 mg every 12 hours. For 10-25 mL/min, 500 mg every 24 hours. For <10 mL/min, 500 mg every 36 hours. A loading dose of 500 mg may be given, followed by maintenance doses.
How to Take
Lidocef 500 mg Capsule should be taken orally. It can be taken with or without food. Taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Cefadroxil exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, thereby interfering with the peptidoglycan synthesis necessary for cell wall integrity.
Pharmacokinetics
Onset
Antibacterial effect typically observed within 1-2 hours after administration.
Excretion
Primarily excreted unchanged by the kidneys, through glomerular filtration and tubular secretion. Over 90% of the dose is excreted in urine within 24 hours.
Half life
Approximately 1.5 to 2 hours in individuals with normal renal function. It can be significantly prolonged in patients with impaired renal function.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Minimally metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefadroxil, any other cephalosporin antibiotics, or to any of the excipients.
- History of severe hypersensitivity (e.g., anaphylaxis) to any other beta-lactam antibiotic (e.g., penicillins).
Drug Interactions
Probenecid
May decrease the renal excretion of cefadroxil, leading to increased and prolonged plasma concentrations.
Live Typhoid Vaccine
Antibiotics may reduce the therapeutic effect of live typhoid vaccine. Administer at least 3 days after stopping cefadroxil.
Anticoagulants (e.g., Warfarin)
Cephalosporins may enhance the anticoagulant effect of warfarin, requiring monitoring of INR.
Loop Diuretics (e.g., Furosemide)
Concomitant use with high doses of cephalosporins may increase the risk of nephrotoxicity.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, abdominal discomfort, and sometimes neuromuscular excitability. Management should be supportive, including gastric lavage or induced emesis. Hemodialysis may be useful in reducing serum concentrations, especially in patients with impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefadroxil is excreted in human milk in small quantities; caution should be exercised when administering to a nursing mother.

WHO Essential Medicine
YesClinical Trials
Cefadroxil has undergone extensive clinical trials to establish its efficacy and safety for various bacterial infections. Its clinical use is supported by decades of data.
Lab Monitoring
- Renal function tests (e.g., serum creatinine, BUN) in patients with pre-existing renal impairment or during prolonged therapy.
- Complete Blood Count (CBC) during prolonged therapy.
- Liver function tests (LFTs) if clinical indications arise.
Doctor Notes
- Assess renal function, especially in elderly patients or those with pre-existing renal impairment, and adjust dosage accordingly.
- Be vigilant for signs of Clostridium difficile-associated diarrhea (CDAD), which can occur even weeks after antibiotic cessation.
- Advise patients to complete the full course of therapy to ensure eradication of infection and prevent resistance.
Patient Guidelines
- Take the medicine for the full prescribed course, even if symptoms improve, to prevent recurrence and development of antibiotic resistance.
- Do not share this medicine with others, even if they have similar symptoms.
- Inform your doctor or pharmacist about any history of allergies, especially to penicillins or other antibiotics.
- If you experience severe diarrhea, especially if bloody or watery, contact your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Cefadroxil generally does not impair the ability to drive or operate machinery. However, if you experience side effects like dizziness or headache, avoid these activities.
Lifestyle Advice
- Maintain adequate hydration by drinking plenty of fluids.
- Avoid alcohol consumption during the course of treatment, as it may exacerbate gastrointestinal side effects.
