Lidocef 125 mg suspension contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including respiratory, urinary tract, and ear infections. It is effective against both Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg to 400 mg daily, given as a single dose or in two divided doses. For suspension, the dosage is generally weight-based for children, typically 8 mg/kg/day.
Elderly
No specific dose adjustment is generally required for elderly patients unless there is severe renal impairment. Renal function should be monitored.
Renal_impairment
For patients with creatinine clearance less than 30 mL/min, the dose should be reduced. For severe renal impairment, consider 200 mg once daily. For patients on chronic ambulatory peritoneal dialysis or hemodialysis, 200 mg once daily is recommended.
How to Take
Shake the suspension well before each use. Can be taken with or without food. Complete the full course of medication as prescribed by the doctor, even if symptoms improve.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis, leading to bacterial cell lysis and death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which are essential for maintaining the integrity of the bacterial cell wall.

Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly affect the extent. Peak plasma concentrations are achieved within 2 to 6 hours after a single oral dose.

Cefixime is not extensively metabolized in the body; no active metabolites have been isolated. It undergoes minor metabolism in the liver.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics (due to potential cross-reactivity).
Drug Interactions
Nifedipine
Concurrent use may increase Cefixime plasma levels, which may enhance its therapeutic effect.
Probenecid
May increase Cefixime concentrations by inhibiting renal excretion, potentially increasing its efficacy and side effects.
Carbamazepine
Increased carbamazepine levels have been reported when taken with Cefixime, requiring monitoring of carbamazepine levels.
Warfarin and other anticoagulants

Live bacterial vaccines (e.g., typhoid vaccine)

Storage
Store unreconstituted powder at room temperature, below 30°C, protected from light and moisture. Keep the container tightly closed. Store reconstituted suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated to remove unabsorbed drug. There is no specific antidote. Hemodialysis or peritoneal dialysis is not effective in removing Cefixime from the body. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Animal studies have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed and the potential benefits outweigh the risks. Cefixime is excreted in small amounts in human breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor before use.
