Description
Overview of the medicine
Lipiles 10 mg Tablet contains Atorvastatin, which is a member of the statin class of medications. It is used to lower cholesterol and other fats (like triglycerides) in the blood. It works by reducing the production of cholesterol by the liver, thereby reducing the risk of cardiovascular diseases.
Uses & Indications
Dosage
Adults
Initial dose 10 mg once daily, adjusted based on LDL-C response, usually ranging from 10 mg to 80 mg once daily. Max dose: 80 mg/day.
Elderly
No dosage adjustment required.
Renal_impairment
No dosage adjustment required.
How to Take
Take orally once daily, with or without food, at any time of day. However, it is often recommended to take it in the evening.
Mechanism of Action
Atorvastatin is a selective, competitive inhibitor of HMG-CoA reductase, the rate-limiting enzyme that catalyzes the conversion of 3-hydroxy-3-methylglutaryl-coenzyme A to mevalonate, a precursor of sterols, including cholesterol. Inhibition of HMG-CoA reductase in the liver leads to decreased cholesterol synthesis and an increase in LDL receptors on liver cells, which enhances the clearance of LDL from the blood.
Pharmacokinetics
Onset
Lipid-lowering effects observed within 2 weeks, maximal at 4 weeks.
Excretion
Primarily excreted in bile after hepatic and/or extrahepatic metabolism. Less than 2% of a dose is recovered in urine.
Half life
Approximately 14 hours (parent drug), active metabolites contribute to plasma HMG-CoA reductase inhibitory activity for 20-30 hours.
Absorption
Rapidly absorbed after oral administration; absolute bioavailability is approximately 12%. Peak plasma concentrations occur within 1 to 2 hours.
Metabolism
Primarily metabolized by cytochrome P450 3A4 (CYP3A4) to ortho- and para-hydroxylated active metabolites and various beta-oxidation products.

Contraindications

Drug Interactions
Warfarin
May cause a slight increase in prothrombin time. Monitor INR.
Colchicine
Cases of myopathy and rhabdomyolysis have been reported. Monitor for symptoms.
Gemfibrozil, other fibrates, Niacin
Increased risk of myopathy/rhabdomyolysis.
Oral contraceptives (e.g., Norethindrone, Ethinyl Estradiol)
Increased AUC of norethindrone and ethinyl estradiol.
Strong CYP3A4 inhibitors (e.g., Cyclosporine, Telithromycin, Clarithromycin, Ritonavir)
Increased plasma concentrations of Atorvastatin, increased risk of myopathy/rhabdomyolysis. Limit atorvastatin dose.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote for Atorvastatin overdose. Treatment should be symptomatic and supportive measures initiated as required. Due to extensive protein binding, hemodialysis is not expected to significantly enhance Atorvastatin clearance.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. Statins can cause fetal harm. Women of childbearing potential should use effective contraception during treatment.
