Description
Overview of the medicine
Lipiles 20 mg Tablet contains Atorvastatin, which belongs to a group of medicines called statins. It is used to lower cholesterol and other fats (triglycerides) in the blood. It also reduces the risk of heart disease and stroke.
Uses & Indications
Dosage
Adults
Initial dose 10-20 mg once daily. May be adjusted up to 80 mg once daily based on LDL-C levels and patient response.
Elderly
No specific dose adjustment required, but caution is advised due to increased risk of myopathy.
Renal_impairment
No dosage adjustment required for patients with renal impairment.
How to Take
Take orally once daily, with or without food, at any time of the day, but preferably at the same time each day.
Mechanism of Action
Atorvastatin competitively inhibits HMG-CoA reductase, an enzyme that catalyzes the early, rate-limiting step in cholesterol biosynthesis. This inhibition reduces cholesterol production in the liver and increases the number of LDL receptors on liver cells, leading to increased uptake and catabolism of LDL.
Pharmacokinetics
Onset
Lipid-lowering effects begin within 2 weeks, with maximum effect generally by 4 weeks.
Excretion
Primarily eliminated in the bile following hepatic and/or extra-hepatic metabolism; less than 2% is recovered in urine.
Half life
Approximately 14 hours (parent drug), 20-30 hours (active metabolites).
Absorption
Rapidly absorbed after oral administration; absolute bioavailability is approximately 14%.
Metabolism
Extensively metabolized by cytochrome P450 3A4 (CYP3A4) to active ortho- and parahydroxylated metabolites and various beta-oxidation products.
Side Effects
Contraindications

Drug Interactions
Colchicine
Increased risk of myopathy, including rhabdomyolysis.
Grapefruit juice
Increases plasma levels of atorvastatin; avoid large quantities.
Gemfibrozil and other fibrates
Increased risk of myopathy/rhabdomyolysis.
Strong CYP3A4 inhibitors (e.g., cyclosporine, clarithromycin, itraconazole)
Increased plasma concentrations of atorvastatin, increasing risk of myopathy/rhabdomyolysis.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. In case of overdose, provide symptomatic and supportive measures. Liver function tests and CK levels should be monitored.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation due to potential harm to the fetus/infant. Women of childbearing potential should use effective contraception.
