Lipiros 10 mg Tablet contains Atorvastatin, which belongs to a class of medicines called 'statins'. It is used to lower cholesterol and other fats (triglycerides) in the blood when a low-fat diet and lifestyle changes alone have failed. It can also be used to reduce the risk of heart disease in certain patients.
Uses & Indications
Dosage
Adults
Initial dose is typically 10-20 mg orally once daily. Dosage may be adjusted at 4-week intervals up to a maximum of 80 mg once daily, based on patient response and tolerability.
Elderly
No specific dosage adjustment required; use with caution due to potential increased risk of myopathy.
Renal_impairment
No dosage adjustment necessary for patients with renal impairment.
How to Take
Take Lipiros 10 mg Tablet orally once daily, with or without food, at any time of day. However, it is advisable to take it at the same time each day to help remember.
Mechanism of Action
Atorvastatin selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. This inhibition reduces cholesterol production in the liver, leading to an increase in hepatic LDL receptors and enhanced uptake and catabolism of LDL-C from the bloodstream.

Onset
Lipid-lowering effects observed within 2 weeks, maximal effects by 4 weeks.
Excretion
Mainly excreted in bile following hepatic and/or extrahepatic metabolism. Less than 2% is recovered in urine.
Half life
Approximately 14 hours for the parent drug; active metabolites contribute to an effective half-life of 20-30 hours.
Absorption
Rapidly absorbed after oral administration; absolute bioavailability is approximately 14%. Peak plasma concentrations achieved within 1-2 hours.
Metabolism
Primarily metabolized by cytochrome P450 3A4 (CYP3A4) to active ortho- and parahydroxylated metabolites and various beta-oxidation products.

Drug Interactions
Cyclosporine
Increased Atorvastatin exposure; avoid co-administration or monitor closely.
Gemfibrozil, other Fibrates, Niacin
Increased risk of myopathy/rhabdomyolysis when co-administered with Atorvastatin.
Strong CYP3A4 Inhibitors (e.g., Itraconazole, Ketoconazole, Clarithromycin, HIV Protease Inhibitors)
May significantly increase Atorvastatin plasma concentrations, increasing risk of myopathy/rhabdomyolysis.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific treatment for Atorvastatin overdose. In case of overdose, general supportive measures should be instituted, and liver function should be monitored. Hemodialysis is unlikely to significantly enhance Atorvastatin clearance.
Pregnancy & Lactation
Atorvastatin is contraindicated during pregnancy and breastfeeding due to potential fetal harm and excretion into breast milk.
