Overview of the medicine
Lipiros 5 mg Tablet contains Atorvastatin, which belongs to a group of medicines called statins. It is used to lower high cholesterol and triglycerides in the blood, reducing the risk of heart attack and stroke.
Uses & Indications
Dosage
Adults
Initial dose is typically 10 mg once daily. Dosage can be adjusted from 5 mg to 80 mg once daily depending on patient response and treatment goal. The 5 mg dose may be used for mild hypercholesterolemia or as a maintenance dose for some patients.
Elderly
No dosage adjustment required based on age, but caution should be exercised due to increased risk of myopathy.
Renal_impairment
No dosage adjustment required in patients with renal impairment.
How to Take
Lipiros tablet should be taken orally once daily, with or without food, at any time of day. It is generally recommended to take it at the same time each day to help remember doses.
Mechanism of Action
Atorvastatin selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. This inhibition leads to a decrease in hepatic cholesterol synthesis and an increase in the number of hepatic LDL receptors, thereby increasing the uptake and catabolism of LDL-C.
Pharmacokinetics
Onset
Lipid-lowering effects observed within 2 weeks, with maximal response usually within 4 weeks.
Excretion
Primarily excreted in the bile following hepatic and/or extrahepatic metabolism. Less than 2% is recovered in urine.
Half life
Approximately 14 hours for the parent drug; active metabolites contribute to a longer half-life of 20-30 hours for HMG-CoA reductase inhibitory activity.
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations reached within 1-2 hours. Absolute bioavailability is approximately 14%.
Metabolism
Extensively metabolized by cytochrome P450 3A4 (CYP3A4) to ortho- and para-hydroxylated derivatives and various beta-oxidation products, which are also pharmacologically active.
Side Effects
Contraindications
- •Active liver disease or unexplained persistent elevations of serum transaminases.
- •Pregnancy and lactation.
- •Hypersensitivity to Atorvastatin or any component of the tablet.
Drug Interactions
Niacin
Increased risk of myopathy/rhabdomyolysis.
Warfarin
May cause a small increase in INR; careful monitoring advised.
Colchicine
Increased risk of myopathy.
Oral contraceptives
Increases AUC of norethindrone and ethinyl estradiol.
Gemfibrozil and other fibrates
Increased risk of myopathy/rhabdomyolysis.
Strong CYP3A4 inhibitors (e.g., cyclosporine, telithromycin, itraconazole, protease inhibitors)
Increased risk of myopathy/rhabdomyolysis due to increased atorvastatin plasma concentrations. Dose reduction of atorvastatin may be necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific treatment for Atorvastatin overdose. In the event of an overdose, the patient should be treated symptomatically, and supportive measures instituted as required. Due to extensive drug binding to plasma proteins, hemodialysis is not expected to significantly enhance atorvastatin clearance.
Pregnancy & Lactation
Atorvastatin is contraindicated during pregnancy and breastfeeding. Women of childbearing potential should use effective contraception during treatment. If pregnancy occurs, treatment should be discontinued immediately.
