Description
Overview of the medicine
Lipostat 10 mg Tablet is a medication used to lower high cholesterol and triglycerides in the blood. It belongs to a group of drugs called statins. It works by blocking an enzyme necessary for cholesterol production, thereby reducing the risk of heart disease and stroke.
Uses & Indications
Dosage
Adults
Initial dose 10-20 mg once daily, usually in the evening. Doses may be adjusted every 4 weeks to a maximum of 80 mg daily. For patients with high cardiovascular risk, a starting dose of 40 mg daily may be considered.
Elderly
No dose adjustment required, but caution is advised. Doses greater than 40 mg daily should be used with caution.
Renal_impairment
For severe renal impairment (creatinine clearance < 30 mL/min), doses greater than 10 mg daily should be carefully considered and, if necessary, implemented cautiously.
How to Take
Take orally once daily in the evening, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Simvastatin is a competitive inhibitor of HMG-CoA reductase, the enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. By inhibiting this enzyme, simvastatin reduces cholesterol synthesis in the liver and decreases the amount of LDL-cholesterol in the blood.
Pharmacokinetics
Onset
Lipid-lowering effects observed within 3 days, maximum effect typically at 4-6 weeks.
Excretion
Primarily excreted in the feces (approx. 60%), with a smaller amount excreted in the urine (approx. 13%).
Half life
About 2-3 hours for the active metabolites.
Absorption
Well absorbed from the GI tract (approx. 60-85%), but undergoes extensive first-pass metabolism, resulting in low systemic bioavailability (<5%).
Metabolism
Extensively metabolized in the liver by CYP3A4 to active beta-hydroxy acid metabolite and other inactive metabolites.
Side Effects
Contraindications
- •Active liver disease or unexplained persistent elevations of serum transaminases
- •Pregnancy and lactation
- •Concomitant use with strong CYP3A4 inhibitors (e.g., itraconazole, ketoconazole, erythromycin, clarithromycin, HIV protease inhibitors, nefazodone, large quantities of grapefruit juice)
- •Hypersensitivity to simvastatin or any component of the formulation
Drug Interactions
Warfarin
May potentiate anticoagulant effect, leading to increased INR/bleeding risk. Monitor INR closely.
Gemfibrozil, Cyclosporine, Danazol
Increased risk of myopathy/rhabdomyolysis. Avoid concomitant use or limit simvastatin dose.
Amiodarone, Amlodipine, Diltiazem, Verapamil
Increased risk of myopathy. Limit simvastatin dose to 20 mg daily.
Strong CYP3A4 inhibitors (e.g., itraconazole, ketoconazole, erythromycin, clarithromycin, protease inhibitors, nefazodone)
Increased risk of myopathy/rhabdomyolysis due to increased simvastatin plasma concentrations. Contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Treatment should be symptomatic and supportive. Gastric lavage and supportive measures may be considered. Closely monitor liver function and creatine kinase levels.
Pregnancy & Lactation
Contraindicated during pregnancy and breastfeeding. Statins may cause fetal harm. Women of childbearing potential should use effective contraception. Small amounts of statins are excreted in breast milk.
