Description
Overview of the medicine
Simvastatin is a lipid-lowering medication used to reduce high cholesterol and triglyceride levels, thereby reducing the risk of heart disease and stroke.
Uses & Indications
Dosage
Adults
Usually 20 mg once daily, preferably in the evening. Dose can be adjusted from 5 mg to 80 mg daily based on response.
Elderly
No specific dose adjustment needed, but monitor closely for side effects.
Renal_impairment
For severe renal impairment (CrCl < 30 mL/min), initial dose 5 mg once daily; maximum 10 mg daily.
How to Take
Oral. Take once daily in the evening, with or without food.
Mechanism of Action
Simvastatin inhibits HMG-CoA reductase, an enzyme critical for cholesterol synthesis in the liver, leading to decreased cholesterol production.
Pharmacokinetics
Onset
Lipid-lowering effects typically seen within 2 weeks, maximal by 4-6 weeks.
Excretion
Primarily excreted in the feces via biliary excretion; a small amount is excreted in urine.

Side Effects
Contraindications

- •Concomitant use with strong CYP3A4 inhibitors (e.g., itraconazole, ketoconazole, erythromycin, clarithromycin, HIV protease inhibitors, nefazodone, cobicistat)
Drug Interactions
Warfarin
May potentiate the anticoagulant effect of warfarin, increasing bleeding risk. INR monitoring is recommended.
Gemfibrozil, Cyclosporine, Danazol
Increased risk of myopathy.
Amiodarone, Verapamil, Diltiazem, Amlodipine
Simvastatin dose should not exceed 20 mg/day (for amiodarone, verapamil, diltiazem) or 40 mg/day (for amlodipine).
Strong CYP3A4 inhibitors (e.g., itraconazole, ketoconazole, clarithromycin, HIV protease inhibitors)
Increased risk of myopathy and rhabdomyolysis. Concomitant use is contraindicated.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Symptomatic and supportive measures should be taken in case of overdose.
Pregnancy & Lactation
Contraindicated in pregnancy and breastfeeding due to potential fetal harm (teratogenicity) and excretion in breast milk.
