Description
Overview of the medicine
Lorastine 5 mg Suspension contains Loratadine, a non-drowsy antihistamine used to relieve symptoms of seasonal and perennial allergic rhinitis, such as sneezing, runny nose, itchy nose or throat, and watery eyes. It is also effective in treating symptoms of chronic idiopathic urticaria (hives), including itching and rash.
Uses & Indications
Dosage
Adults
10 mg (10 mL of 5 mg/5 mL suspension) once daily.
Elderly
No dosage adjustment is generally required in elderly patients with normal renal and hepatic function.
Renal_impairment
For patients with severe renal impairment (creatinine clearance < 30 mL/min), a starting dose of 5 mg every other day is recommended.
Hepatic_impairment
For patients with hepatic impairment, a starting dose of 5 mg every other day is recommended.
Children_2_to_5_years
5 mg (5 mL of 5 mg/5 mL suspension) once daily.
Children_6_years_and_older
10 mg (10 mL of 5 mg/5 mL suspension) once daily.
How to Take
Take orally, with or without food. Use a calibrated measuring spoon or cup for accurate dosing of suspension.
Mechanism of Action
Loratadine is a tricyclic antihistamine with selective peripheral H1-receptor antagonist activity. It prevents histamine from binding to H1 receptors, thereby inhibiting the inflammatory effects of histamine, such as vasodilation, increased vascular permeability, and itching. It has minimal sedative effects due to poor penetration of the blood-brain barrier.

Approximately 40% in urine and 42% in feces over a 10-day period, mainly as conjugated metabolites.
Half life
Loratadine: 8.4 hours (range 3-20 hours). Desloratadine (active metabolite): 27.8 hours (range 8.8-92 hours).
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached in 1-1.5 hours.
Metabolism
Extensively metabolized in the liver primarily by CYP3A4 and CYP2D6 enzymes to its major active metabolite, desloratadine.

Contraindications
- Hypersensitivity to Loratadine or any component of the formulation.
- Use in children under 2 years of age (unless directed by a physician).
Drug Interactions
Cimetidine
May increase plasma concentrations of Loratadine and desloratadine, potentially increasing side effects. Use with caution.
Erythromycin
May increase plasma concentrations of Loratadine and desloratadine, potentially increasing side effects. Use with caution.
Ketoconazole
May increase plasma concentrations of Loratadine and desloratadine, potentially increasing side effects. Use with caution.
Grapefruit Juice
May increase absorption and plasma levels of Loratadine.

Overdose
Symptoms of overdose include somnolence, tachycardia, and headache. In case of overdose, symptomatic and supportive treatment is recommended. Gastric lavage may be considered. Loratadine is not effectively removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of fetal harm, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Loratadine and its active metabolite are excreted into breast milk; therefore, caution should be exercised when administering to a nursing mother.
