Description
Overview of the medicine
Loratadine is a non-drowsy antihistamine used for relief of allergy symptoms such as sneezing, runny nose, itchy eyes, and hives.
Uses & Indications
Dosage
Adults
10 mg (10 ml of 5 mg/5 ml suspension) once daily.
Elderly
No dosage adjustment generally needed, but caution with severe hepatic impairment.
Renal_impairment
For severe renal impairment (creatinine clearance < 30 ml/min), initial dose 5 mg (5 ml of 5 mg/5 ml suspension) every other day.
Children_2_5_years
5 mg (5 ml of 5 mg/5 ml suspension) once daily.
Children_6_years_and_older
10 mg (10 ml of 5 mg/5 ml suspension) once daily.
How to Take
Administer orally, with or without food. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Loratadine selectively blocks peripheral H1-receptors, thereby inhibiting the effects of histamine which is responsible for allergic symptoms.
Pharmacokinetics
Onset
Onset of action within 1-3 hours.
Excretion
Approximately 40% in urine and 42% in feces over 10 days, mainly as conjugated metabolites.
Half life
Parent drug: approximately 8.4 hours; active metabolite (desloratadine): approximately 28 hours.
Absorption
Rapidly absorbed after oral administration. Food may slightly delay absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4 and CYP2D6, to its major active metabolite, desloratadine.

Contraindications
- Hypersensitivity to loratadine, desloratadine, or any component of the formulation.
Drug Interactions
Cimetidine
May increase plasma concentrations of loratadine and desloratadine, but generally without clinically significant changes in ECG.
Erythromycin
May increase plasma concentrations of loratadine and desloratadine, but generally without clinically significant changes in ECG.
Ketoconazole
May increase plasma concentrations of loratadine and desloratadine, but generally without clinically significant changes in ECG.

Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.

Symptoms of overdose include somnolence, tachycardia, and headache. Management is symptomatic and supportive; hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Loratadine is excreted in breast milk; therefore, use is not recommended during breastfeeding.
