Lorazepam is a benzodiazepine used to treat anxiety disorders, insomnia, certain types of seizures, and for pre-anesthetic sedation. The 10 mg tablet is typically reserved for severe cases or specific indications.
Uses & Indications
Dosage
Adults
Anxiety: 1-4 mg/day in 2 or 3 divided doses. Insomnia: 2-4 mg at bedtime. Pre-medication: 2-4 mg the night before or 1-2 hours before procedure. For 10mg tablet, specific severe conditions may warrant, consult physician.
Elderly
Initially 1-2 mg/day in divided doses, adjusted as tolerated and needed due to increased sensitivity and slower elimination.
Renal_impairment
No dose adjustment is generally necessary, but caution is advised in severe impairment. Monitor closely for sedation.
How to Take
Take orally, with or without food. Swallow the tablet whole with water. Do not chew or crush.
Mechanism of Action
Lorazepam enhances the effect of gamma-aminobutyric acid (GABA), a neurotransmitter that reduces brain activity. This results in sedative, hypnotic, anxiolytic, anticonvulsant, and muscle relaxant properties.

Side Effects
Contraindications

Drug Interactions
Clozapine
Increased risk of sedation, hypotension, and respiratory depression.
Scopolamine
Increased incidence of sedation, hallucinations, and irrational behavior.
Valproate, Probenecid
May increase lorazepam plasma concentrations and prolong its half-life, requiring dose adjustment.
Opioids, alcohol, other CNS depressants
Increased risk of profound sedation, respiratory depression, coma, and death.
Storage
Store at controlled room temperature (20°C to 25°C), away from moisture, heat, and direct light. Keep out of reach of children.
Overdose
Symptoms: Drowsiness, confusion, lethargy, ataxia, hypotonia, hypotension, respiratory depression, and coma. Management: Supportive care, monitor vital signs, maintain airway. Flumazenil (benzodiazepine antagonist) may be used but with caution due to risk of seizures.
Pregnancy & Lactation
Pregnancy Category D: May cause fetal harm. Not recommended for use during pregnancy, especially during the first trimester. Lactation: Excreted in breast milk and may cause sedation or feeding difficulties in infants. Use with caution or avoid.
