Overview of the medicine
Losapot-Plus 50 mg Tablet is a combination medicine containing Losartan Potassium and Hydrochlorothiazide, used to treat high blood pressure (hypertension). Losartan is an angiotensin II receptor blocker (ARB) which relaxes blood vessels, and Hydrochlorothiazide is a thiazide diuretic which helps the body get rid of excess water and salt.
Uses & Indications
Dosage
Adults
Usually 1 tablet (Losartan Potassium 50 mg + Hydrochlorothiazide 12.5 mg) once daily. The dose may be increased if needed after 3-4 weeks.
Elderly
No initial dosage adjustment is typically required, but monitoring of renal function and electrolytes is recommended.
Renal_impairment
Not recommended in patients with severe renal impairment (creatinine clearance <30 mL/min).
How to Take
Take orally, with or without food, usually once daily in the morning.
Mechanism of Action
Losartan, an ARB, selectively blocks the binding of angiotensin II to the AT1 receptor, inhibiting vasoconstriction and aldosterone secretion, leading to reduced blood pressure. Hydrochlorothiazide, a thiazide diuretic, increases the excretion of sodium, chloride, and water by inhibiting the sodium-chloride symporter in the distal convoluted tubule, thereby reducing plasma volume and blood pressure.

Onset
Losartan: 1-2 hours; Hydrochlorothiazide: 2 hours.
Excretion
Losartan and its active metabolite are excreted via urine (35%) and feces (60%). Hydrochlorothiazide is primarily excreted unchanged in urine.
Half life
Losartan: approximately 2 hours; active metabolite: 6-9 hours. Hydrochlorothiazide: 5.6-14.8 hours.
Absorption
Losartan is well absorbed orally, approximately 33% bioavailability. Hydrochlorothiazide is absorbed (60-80%) from the GI tract.
Metabolism
Losartan undergoes significant first-pass metabolism to an active carboxylic acid metabolite. Hydrochlorothiazide is not metabolized.

Contraindications
- Hypersensitivity to Losartan, Hydrochlorothiazide, or sulfonamide-derived drugs
- Anuria (inability to produce urine)
- Severe renal or hepatic impairment
- Pregnancy (second and third trimesters)
- Concomitant use with aliskiren in diabetic patients
Drug Interactions
NSAIDs
May reduce the hypotensive effect and increase the risk of renal impairment.
Lithium
May increase serum lithium levels and toxicity.
Corticosteroids
Increased electrolyte depletion, particularly hypokalemia.
Other antihypertensives
Additive hypotensive effect.
Alcohol/Barbiturates/Narcotics
May potentiate orthostatic hypotension.
Potassium-sparing diuretics/Potassium supplements
May increase serum potassium levels (risk of hyperkalemia).
