Description
Overview of the medicine
Losartan 25 mg tablet is an Angiotensin II Receptor Blocker (ARB) used to treat high blood pressure (hypertension) and to protect the kidneys from damage due to diabetes.
Uses & Indications
Dosage
Adults
Initial dose is usually 50 mg once daily. For Losartan 25 mg, it might be used as a starting dose or for dose titration, usually 25 mg once daily.
Elderly
No initial dose adjustment required for most elderly patients, but consider lower starting dose (e.g., 25 mg once daily) for those over 75 years.
Renal_impairment
No initial dose adjustment required in patients with mild to moderate renal impairment. For severe impairment, consider lower starting dose (e.g., 25 mg once daily).
How to Take
Losartan tablets can be taken with or without food, usually once daily. It is recommended to take the tablet at the same time each day.
Mechanism of Action
Losartan selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues (e.g., vascular smooth muscle, adrenal gland). This blockade inhibits the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to decreased blood pressure.
Pharmacokinetics
Onset
Within 6 hours (full effect within 3-6 weeks)
Excretion
Approximately 35% in urine and 60% in feces (as unchanged drug and metabolites).
Half life
Losartan: 2 hours; active metabolite (E-3174): 6-9 hours
Absorption
Rapidly absorbed from the gastrointestinal tract; undergoes extensive first-pass metabolism. Bioavailability is approximately 33%.
Metabolism
Hepatic (primarily by CYP2C9 and CYP3A4) to an active carboxylic acid metabolite (E-3174) and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Losartan or any component of the formulation
- Pregnancy (especially 2nd and 3rd trimesters)
- Concomitant use with aliskiren in patients with diabetes mellitus
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity have been reported.
Aliskiren
Concomitant use with aliskiren is contraindicated in patients with diabetes or renal impairment.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of Losartan and increase the risk of renal impairment.
Potassium-sparing diuretics (e.g., spironolactone, amiloride) or potassium supplements
May lead to increased serum potassium levels (hyperkalemia).
Storage
Store at room temperature (below 30°C), protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension (low blood pressure), tachycardia (rapid heartbeat), and possibly bradycardia (slow heartbeat). Treatment is symptomatic and supportive, focusing on maintaining blood pressure and hydration.
Pregnancy & Lactation
Contraindicated in the 2nd and 3rd trimesters of pregnancy due to fetal toxicity. Use during lactation is not recommended as it is unknown if Losartan is excreted in human milk.
