Description
Overview of the medicine
Losartan 50 mg tablet is an Angiotensin II Receptor Blocker (ARB) used to treat high blood pressure (hypertension), reduce the risk of stroke in patients with high blood pressure and an enlarged heart, and manage kidney disease in type 2 diabetic patients.
Uses & Indications
Dosage
Adults
Initial dose: 50 mg once daily. Maintenance dose: 25-100 mg once daily, adjusted based on blood pressure response.
Elderly
Generally no initial dosage adjustment needed, but monitor closely, especially in those with intravascular volume depletion.
Renal_impairment
No initial dosage adjustment needed for mild-to-moderate renal impairment. For severe renal impairment or dialysis patients, a lower initial dose of 25 mg once daily may be considered.
How to Take
Take orally, with or without food, usually once daily. Swallow the tablet whole with a glass of water.
Mechanism of Action
Losartan is an angiotensin II receptor blocker (ARB) that selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues (e.g., vascular smooth muscle, adrenal gland). This blockade inhibits the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased sodium and water retention, and ultimately a reduction in blood pressure.
Pharmacokinetics
Onset
1-2 hours for antihypertensive effect. Maximum effect in 3-6 weeks.
Excretion
Approximately 35% of the dose is excreted in the urine and 58% in the feces via bile.
Half life
Losartan: 2 hours; active metabolite (E-3174): 6-9 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is about 33%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9 and CYP3A4, to an active carboxylic acid metabolite (E-3174) and several inactive metabolites.
Side Effects
Contraindications
- Pregnancy (especially 2nd and 3rd trimesters)
- Hypersensitivity to losartan or any component of the formulation
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73m²)
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Contraindicated in diabetic patients or those with renal impairment (GFR < 60) due to increased risk of renal impairment, hyperkalemia, and hypotension.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce antihypertensive effect and increase risk of renal impairment.
Diuretics (potassium-sparing) / Potassium supplements
Increased risk of hyperkalemia.

Symptoms of overdose include hypotension (low blood pressure) and tachycardia (rapid heartbeat); bradycardia (slow heartbeat) may occur from parasympathetic stimulation. Treatment is symptomatic and supportive, focusing on maintaining cardiovascular stability.

Pregnancy: Contraindicated during the second and third trimesters due to risk of fetal injury and death. Discontinue immediately if pregnancy is detected. Lactation: Not recommended during breastfeeding due to potential risk to the nursing infant.
