Description
Overview of the medicine
Losartan is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension), diabetic nephropathy in type 2 diabetes with hypertension, and to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy.
Uses & Indications
Dosage
Adults
The usual starting dose for hypertension is 50 mg once daily. Doses can range from 25 mg to 100 mg daily. For Losita 10 mg, it may be used for titration or in sensitive patients, typically once daily. Max 100 mg/day.
Elderly
No initial dose adjustment is generally required, but consider a lower starting dose (e.g., 25 mg) in some elderly patients.
Renal_impairment
No initial dose adjustment is required for patients with mild to moderate renal impairment. A lower starting dose (e.g., 25 mg) should be considered for patients with severe renal impairment or on dialysis.
How to Take
Take Losita 10 mg Tablet orally, once daily, with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Losartan selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues, including vascular smooth muscle and the adrenal gland. This prevents the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation, reduced fluid retention, and decreased blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect begins within 6 hours, with full effect taking 3-6 weeks.
Excretion
Approximately 35% excreted in urine and 60% in feces, primarily as unchanged drug and its active metabolite.
Half life
Losartan: approximately 2 hours. Active metabolite (E-3174): approximately 6-9 hours.
Absorption
Well absorbed from the gastrointestinal tract, undergoes significant first-pass metabolism. Bioavailability is approximately 33%. Peak plasma concentrations of Losartan are reached in about 1 hour and of its active metabolite (E-3174) in about 3-4 hours.
Metabolism
Undergoes extensive hepatic first-pass metabolism, primarily by CYP2C9 and CYP3A4 enzymes, to its active carboxylic acid metabolite (E-3174).
Side Effects
Contraindications
- Hypersensitivity to Losartan or any component of the formulation
- Pregnancy (especially second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes
Drug Interactions
Lithium
May increase serum lithium concentrations and lead to lithium toxicity.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment, especially in dehydrated or elderly patients.
Potassium-sparing diuretics/Potassium supplements
May increase the risk of hyperkalemia (high potassium levels).

Store below 30°C in a cool, dry place, away from direct sunlight and moisture. Keep out of reach of children.

Symptoms of overdose include hypotension (low blood pressure) and tachycardia (rapid heartbeat); bradycardia (slow heartbeat) could occur from parasympathetic stimulation. Management involves symptomatic and supportive treatment.
