Losita 5 mg Tablet is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to help protect the kidneys from damage due to diabetes.
Uses & Indications
Dosage
Adults
Typical initial dose for hypertension is 25-50 mg once daily. Max dose usually 100 mg daily. A 5 mg dose may be used in specific cases for very sensitive patients or as a starting point under strict medical supervision.
Elderly
No specific dose adjustment is usually required, but a lower starting dose (e.g., 25 mg) may be considered for frail or volume-depleted elderly patients. A 5 mg dose could be considered for very sensitive elderly patients.
Renal_impairment
No initial dose adjustment generally needed for mild-to-moderate renal impairment. For severe impairment, a lower starting dose (e.g., 25 mg) may be considered. Regular monitoring of renal function and potassium levels is recommended.
How to Take
Losita 5 mg Tablet should be taken orally once daily, with or without food. It is important to take it at the same time each day.
Mechanism of Action
Losita selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues (e.g., vascular smooth muscle, adrenal gland, kidneys, and heart). This prevents the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect usually apparent within 1 week, maximal effect by 3-6 weeks.
Excretion
Approximately 35% excreted in urine and 60% in feces, primarily as metabolites.
Half life
Losita and its active metabolite have elimination half-lives of approximately 2 hours and 6-9 hours, respectively.
Absorption
Well absorbed from the gastrointestinal tract; undergoes significant first-pass metabolism. Bioavailability is approximately 33%.
Metabolism
Extensively metabolized by cytochrome P450 enzymes (CYP2C9 and CYP3A4) to an active carboxylic acid metabolite (E-3174) and other inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to losartan or any component of the formulation
- Pregnancy (second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes mellitus
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Concomitant use is contraindicated in patients with diabetes or renal impairment (GFR < 60 mL/min/1.73 m2) due to increased risk of hypotension, hyperkalemia, and renal impairment.
Diuretics (potassium-sparing)
Increased risk of hyperkalemia.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment.

Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension and tachycardia; bradycardia could occur from parasympathetic stimulation. Treatment is symptomatic and supportive. If ingestion is recent, gastric lavage may be considered. Activated charcoal may be useful.
Pregnancy & Lactation
Pregnancy Category D (2nd and 3rd trimesters): Losita can cause fetal harm when administered to a pregnant woman. Discontinue as soon as pregnancy is detected. It is not known whether Losita is excreted in human breast milk; however, it is advised to avoid breastfeeding during treatment.
