Description
Overview of the medicine
Lotenate (Loteprednol Etabonate) is a corticosteroid medication used to treat various inflammatory conditions of the eye. It works by reducing swelling, redness, and itching.
Uses & Indications
Dosage
Adults
For steroid-responsive inflammatory conditions: Instill one or two drops into the conjunctival sac of the affected eye(s) four times daily. For post-operative inflammation: Instill one or two drops into the conjunctival sac of the operated eye(s) four times daily, beginning 24 hours after surgery and continuing throughout the first 2 weeks of the post-operative period.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
No specific dose adjustment is required as systemic absorption is minimal.
How to Take
For ophthalmic use only. Shake well before use. Do not touch the dropper tip to any surface to avoid contamination.
Mechanism of Action
Loteprednol etabonate is a 'site-specific' corticosteroid that penetrates quickly into target tissues (ocular cells) and undergoes rapid biotransformation to inactive metabolites after exerting its therapeutic effect, minimizing systemic side effects. It inhibits the release of inflammatory mediators like prostaglandins and leukotrienes.
Pharmacokinetics
Onset
Onset of anti-inflammatory action within hours, with peak effect in a few days.
Excretion
Primarily excreted as inactive metabolites via urine.
Half life
Not definitively characterized for topical ophthalmic use due to minimal systemic absorption.
Absorption
Minimal systemic absorption following topical ophthalmic administration. Plasma concentrations are generally below the limit of quantitation (1 ng/mL).
Metabolism
Rapidly metabolized by esterases in ocular tissues to inactive carboxylic acid metabolites.
Side Effects
Contraindications
- •Hypersensitivity to loteprednol etabonate or any component of the medication.
- •Viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella.
- •Mycobacterial infections of the eye.
- •Fungal diseases of ocular structures.
Drug Interactions
0
No significant systemic drug interactions are expected due to minimal systemic absorption.
1
May interact with topical ophthalmic anticholinergics by increasing IOP.
Storage
Store upright at 15°C to 25°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Acute ocular overdose is unlikely to cause systemic toxicity due to limited absorption. If overdose occurs, flush the eye with warm water. Management is supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is not known whether topical ophthalmic administration of corticosteroids could result in sufficient systemic absorption to produce detectable quantities in human milk. Exercise caution when administered to a nursing woman.
