Overview of the medicine
Lotenate-Plus is an ophthalmic suspension combining a corticosteroid (Loteprednol Etabonate) and an aminoglycoside antibiotic (Tobramycin). It is used to treat ocular inflammation where bacterial infection is present or there is a risk of bacterial infection.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the conjunctival sac of the affected eye(s) 4-6 times daily. During the initial 24 to 48 hours, the dosage may be increased to every 1-2 hours.
Elderly
No specific dosage adjustment is generally required for elderly patients.
Renal_impairment
No specific dosage adjustment is required for patients with renal impairment due to minimal systemic absorption.
How to Take
For ophthalmic use only. Shake well before use. Do not touch the dropper tip to any surface to avoid contamination. Remove contact lenses before administration and wait at least 15 minutes before reinserting.
Mechanism of Action
Loteprednol Etabonate is a corticosteroid that suppresses the inflammatory response through various mechanisms, including inhibition of prostaglandin and leukotriene synthesis. Tobramycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by irreversibly binding to the 30S ribosomal subunit, leading to bacterial cell death.
Pharmacokinetics
Onset
Anti-inflammatory effects can begin within hours, and significant reduction in bacterial load typically observed within days.
Excretion
Excretion of systemically absorbed components is primarily renal, but systemic absorption is minimal with topical ophthalmic use.
Half life
Systemic half-life of Loteprednol Etabonate is short due to rapid metabolism (approximately 1-2 hours). Tobramycin's systemic half-life is around 2-3 hours but systemic levels are negligible with ophthalmic use.
Absorption
Limited systemic absorption following topical ophthalmic administration. Loteprednol is rapidly metabolized in the eye to inactive compounds. Tobramycin also has minimal systemic absorption from the eye.
Metabolism
Loteprednol Etabonate undergoes rapid metabolism to inactive carboxylic acid metabolites in the eye. Tobramycin is not significantly metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Loteprednol Etabonate, Tobramycin, or any component of the formulation.
- •Most viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella.
- •Mycobacterial infection of the eye.
- •Fungal diseases of ocular structures.
Drug Interactions
Other topical ophthalmic medications
If using more than one topical ophthalmic product, administer them at least 5 minutes apart to avoid washout.
Storage
Store upright at room temperature (15-30°C). Protect from light. Do not freeze.
Overdose
Ophthalmic overdose is unlikely to cause systemic toxicity due to limited systemic absorption. In case of an overdose, flush the eye(s) with lukewarm water.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing woman as it is unknown if it is excreted in human milk. Consult a doctor.
