Description
Overview of the medicine
Lotepro G is a combination medication used to treat high blood pressure. It contains Losartan, an angiotensin II receptor blocker, and Hydrochlorothiazide, a thiazide diuretic.
Uses & Indications
Dosage
Adults
Typically, one tablet (50 mg Losartan/12.5 mg Hydrochlorothiazide) once daily. Dosage may be increased to 100 mg/25 mg daily if needed.
Elderly
No initial dosage adjustment is generally required, but close monitoring of renal function and electrolytes is recommended.
Renal_impairment
Not recommended for patients with severe renal impairment (creatinine clearance < 30 mL/min). Dose adjustment may be needed for moderate impairment.
How to Take
Lotepro G can be taken orally, with or without food. It is usually taken once daily.
Mechanism of Action
Losartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the AT1 receptor. Hydrochlorothiazide is a diuretic that acts by inhibiting sodium reabsorption in the distal convoluted tubules, increasing the excretion of sodium, chloride, and water.
Pharmacokinetics
Onset
Losartan: 1-2 hours. Hydrochlorothiazide: 2 hours.
Excretion
Losartan and its active metabolite are primarily excreted in urine and feces. Hydrochlorothiazide is excreted predominantly unchanged in urine.
Half life
Losartan: approximately 2 hours (active metabolite 6-9 hours). Hydrochlorothiazide: 6-15 hours.
Absorption
Losartan is well absorbed from the gastrointestinal tract, with a bioavailability of about 33%. Hydrochlorothiazide is absorbed moderately (60-80%).
Metabolism
Losartan undergoes significant first-pass metabolism to an active carboxylic acid metabolite. Hydrochlorothiazide is minimally metabolized.
Side Effects
Contraindications
- Hypersensitivity to any component of the product.
- Anuria or severe renal impairment.
- Pregnancy (especially 2nd and 3rd trimesters).
- Concomitant use with aliskiren in patients with diabetes or renal impairment.
Drug Interactions
Lithium
Increases lithium serum concentrations and lithium toxicity.
Alcohol, barbiturates, or narcotics
May potentiate orthostatic hypotension.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the diuretic and antihypertensive effects; risk of worsening renal function.
Potassium-sparing diuretics, potassium supplements, or potassium-containing salt substitutes
May lead to hyperkalemia (high potassium levels).

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension (low blood pressure), tachycardia (rapid heartbeat), and possibly bradycardia (slow heartbeat). Management is symptomatic and supportive, including IV fluids and vasopressors if severe hypotension occurs.
Pregnancy & Lactation
Category D in pregnancy (risk to fetus). Contraindicated in the 2nd and 3rd trimesters. Not recommended during breastfeeding as both drugs are excreted in breast milk.
