Description
Overview of the medicine
Lubicon 24 mcg Capsule contains Lubiprostone, a chloride channel activator used to treat chronic idiopathic constipation (CIC), opioid-induced constipation (OIC) in adults with chronic non-cancer pain, and irritable bowel syndrome with constipation (IBS-C) in women 18 years of age and older.
Uses & Indications
Dosage
Adults
For CIC and OIC: 24 mcg orally twice daily with food and water. For IBS-C (if prescribed 24mcg, typically 8mcg is used): 8 mcg orally twice daily with food and water.
Elderly
No specific dosage adjustment recommended based on age alone. Caution should be exercised due to potential increased susceptibility to adverse effects.
Renal_impairment
No dosage adjustment recommended for mild-to-moderate renal impairment. For severe renal impairment, consider a reduced starting dose.
How to Take
Take orally, with food and water. Swallow the capsule whole; do not break or chew.
Mechanism of Action
Lubiprostone acts locally on the apical membrane of gastrointestinal epithelial cells by activating ClC-2 chloride channels. This increases intestinal fluid secretion and motility, leading to softened stool and increased spontaneous bowel movements.
Pharmacokinetics
Onset
First spontaneous bowel movement within 24-48 hours.
Excretion
Mainly excreted via feces as metabolites; minimal urinary excretion.
Half life
Very short; plasma half-life of active metabolite is approximately 0.9-1.4 hours.
Absorption
Minimally absorbed systemically. Peak plasma concentrations are very low and occur rapidly.
Metabolism
Rapidly metabolized in the stomach and jejunum by carbonyl reductase, with minimal hepatic metabolism.
Side Effects
Contraindications
- Known or suspected mechanical gastrointestinal obstruction.
- Severe diarrhea.
Drug Interactions
Methadone
May decrease the efficacy of Lubiprostone, especially in OIC patients.
Other laxatives
Concurrent use not generally recommended without medical advice.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for Lubiprostone overdose. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is unknown if Lubiprostone is excreted in human milk; use with caution in breastfeeding women.
