Description
Overview of the medicine
Lubicon 8 mcg capsule contains Lubiprostone, a prostaglandin E1 derivative, used to treat chronic idiopathic constipation (CIC) and irritable bowel syndrome with constipation (IBS-C) in adults. It works by increasing fluid secretion into the intestines, softening stool, and promoting bowel movements.
Uses & Indications
Dosage
Adults
For CIC and OIC: 24 mcg orally twice daily with food and water. For IBS-C: 8 mcg orally twice daily with food and water.
Elderly
No specific dosage adjustment is required based on age alone, but caution is advised due to potential for comorbidities.
Renal_impairment
No dosage adjustment is required in patients with renal impairment.
How to Take
Lubicon capsules should be taken orally twice daily with food and water. Swallow the capsule whole; do not break or chew.
Mechanism of Action
Lubiprostone is a locally acting chloride channel activator that specifically activates ClC-2 chloride channels on the apical membrane of gastrointestinal epithelial cells. This activation leads to an increase in intestinal fluid secretion, which softens the stool and enhances motility, facilitating spontaneous bowel movements.
Pharmacokinetics
Onset
First spontaneous bowel movement within 24-48 hours, with continued improvement over time.
Excretion
Mainly excreted via feces and urine as metabolites.
Half life
Approximately 0.9 to 1.4 hours for active metabolites.
Absorption
Minimal systemic absorption; plasma concentrations are typically below the quantifiable limit (50 pg/mL).
Metabolism
Primarily metabolized by aldoketo reductases in the gastrointestinal tract and liver, not by the cytochrome P450 system.
Side Effects
Contraindications
- Known or suspected mechanical gastrointestinal obstruction.
- Hypersensitivity to lubiprostone or any component of the capsule.
Drug Interactions
Methadone
May reduce the efficacy of lubiprostone, particularly in opioid-induced constipation.
Antidiarrheals
Concomitant use with antidiarrheal agents is not recommended as it may counteract the effects of lubiprostone.
Storage
Store at controlled room temperature (20°C to 25°C), away from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose are generally an exaggeration of common side effects, primarily diarrhea and nausea. Treatment is symptomatic and supportive. No specific antidote is available.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. It is unknown if lubiprostone is excreted in human milk; caution is advised when administered to a nursing woman.
