Overview of the medicine
Lubiprostone is a chloride channel activator used to treat chronic idiopathic constipation, irritable bowel syndrome with constipation (IBS-C), and opioid-induced constipation in adults.
Uses & Indications
Dosage
Adults
For Chronic Idiopathic Constipation (CIC) and Opioid-Induced Constipation (OIC): 24 mcg orally twice daily with food and water. For Irritable Bowel Syndrome with Constipation (IBS-C): 8 mcg orally twice daily with food and water.
Elderly
No dosage adjustment generally required, but caution due to potential for increased sensitivity. Monitor for efficacy and adverse effects.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Use with caution in severe impairment. Monitor for adverse effects.
How to Take
Take orally with food and water to reduce the incidence of nausea. Do not crush, chew, or break the capsule.
Mechanism of Action
Lubiprostone activates ClC-2 chloride channels on the apical membrane of gastrointestinal epithelial cells, leading to an efflux of chloride and fluid into the intestinal lumen. This softens stool, increases motility, and facilitates spontaneous bowel movements.

Onset
Typically within 24-48 hours for chronic idiopathic constipation.
Excretion
Primarily via feces (approximately 60%) and urine (approximately 3%) as metabolites.

Approximately 0.9 to 1.4 hours for active metabolite.
Absorption
Minimally absorbed systemically after oral administration. Peak plasma concentrations are very low and often below the limit of quantification.
Metabolism

Methadone
May decrease the effectiveness of lubiprostone, consider alternative laxatives or higher lubiprostone dose.
