Description
Overview of the medicine
Lubiprostone is a chloride channel activator used to treat chronic idiopathic constipation, irritable bowel syndrome with constipation (IBS-C) in women, and opioid-induced constipation (OIC) in adults.
Uses & Indications
Dosage
Adults
CIC: 24 mcg orally twice daily. IBS-C (women): 8 mcg orally twice daily. OIC: 24 mcg orally twice daily.
Elderly
No specific dosage adjustment based on age alone, but monitor for side effects.
Renal_impairment
For severe renal impairment (CrCl <29 mL/min): reduce dose to 8 mcg twice daily. No adjustment for mild to moderate impairment.
How to Take
Take orally with food and water. Swallow the capsule whole; do not chew or crush it.
Mechanism of Action
It activates chloride channels (ClC-2) in the apical membrane of the gastrointestinal tract, increasing intestinal fluid secretion, which softens the stool and promotes spontaneous bowel movements.
Pharmacokinetics
Onset
First spontaneous bowel movement often occurs within 24-48 hours.
Excretion
Primarily eliminated via feces (60%) and urine (30%) as metabolites.
Half life
Very short, parent drug is rapidly metabolized.
Absorption
Minimal systemic absorption, primarily acts locally in the gut.
Metabolism
Rapidly metabolized in the stomach and jejunum via reduction/oxidation by carbonyl reductase.
Side Effects
Contraindications
- Known or suspected mechanical gastrointestinal obstruction.
- Severe diarrhea.
Drug Interactions
Opioids
May reduce the efficacy of lubiprostone by counteracting its effects on gut motility and fluid secretion. Consider increased dosage or alternative treatments.
Storage
Store below 30°C (86°F). Protect from moisture and light. Keep out of reach of children.
Overdose
In case of overdose, provide symptomatic and supportive treatment. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefit justifies the potential risk to the fetus. It is unknown if lubiprostone is excreted in human milk; caution is advised when administered to a nursing mother.
