Description
Overview of the medicine
Lucon 50 mg Capsule contains Fluconazole, an antifungal medication used to treat a variety of fungal infections, including vaginal, oral, esophageal, and systemic candidiasis, as well as cryptococcal meningitis.
Uses & Indications
Dosage
Adults
Vaginal candidiasis: 150 mg single oral dose. Oropharyngeal candidiasis: 50 mg daily for 7-14 days. Esophageal candidiasis: 50-400 mg daily for 14-30 days. Cryptococcal meningitis: Loading dose 400 mg, then 200-400 mg daily for 10-12 weeks.
Elderly
Similar to adult dose, but dosage adjustment may be necessary in elderly patients with impaired renal function. Monitor creatinine clearance.
Renal_impairment
For patients with creatinine clearance (CrCl) less than 50 mL/min, the dose should be reduced by 50% after the initial loading dose (if applicable).
How to Take
Lucon capsules can be taken with or without food. Swallow the capsule whole with a glass of water.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450-dependent 14-alpha-demethylase, an enzyme essential for the synthesis of ergosterol, a vital component of fungal cell membranes. This disruption leads to increased cellular permeability and eventual fungal cell death.
Pharmacokinetics
Onset
Onset of antifungal action varies depending on the type and severity of infection, typically within days to weeks for resolution of symptoms.
Excretion
Primarily excreted unchanged in the urine (approximately 80% of the dose), with a small amount excreted in feces (11%).
Half life
Approximately 30 hours, which allows for once-daily dosing for many indications.
Absorption
Well absorbed orally; bioavailability is typically >90% of an intravenously administered dose. Peak plasma concentrations occur within 1-2 hours.
Metabolism
Minimally metabolized by the liver (approximately 11% of the dose); it is a potent inhibitor of CYP2C9 and a moderate inhibitor of CYP3A4 and CYP2C19.
Side Effects
Contraindications
- Hypersensitivity to fluconazole, other azole antifungals, or any excipients of the product.
- Co-administration with drugs known to prolong the QT interval and metabolized by CYP3A4, such as terfenadine, cisapride, astemizole, pimozide, or quinidine, in patients receiving multiple doses of fluconazole.
Drug Interactions
Warfarin
Increased prothrombin time; monitor INR.
Phenytoin
Increased phenytoin levels; monitor levels.
Rifampicin
Decreased fluconazole levels; consider increasing fluconazole dose.
Hydrochlorothiazide
Increased fluconazole plasma concentrations.
Cyclosporine, Tacrolimus
Increased levels of immunosuppressants; monitor levels.
Oral Hypoglycemics (sulfonylureas)
Increased blood glucose lowering effect; monitor blood sugar.
Statins (e.g., Atorvastatin, Simvastatin)
Increased risk of myopathy and rhabdomyolysis; monitor for muscle pain.
Short-acting benzodiazepines (e.g., Midazolam)
Increased AUC and half-life of benzodiazepines.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive treatment may be adequate. Hemodialysis can reduce plasma concentrations by approximately 50% in 3 hours.
Pregnancy & Lactation
Pregnancy Category D for chronic use, Category C for single-dose vaginal candidiasis. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Fluconazole is excreted in breast milk; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- Liver function tests (ALT, AST, alkaline phosphatase, bilirubin) for patients on prolonged therapy.
- Renal function tests (creatinine, BUN) for patients with renal impairment.
Doctor Notes
- Be aware of potential drug interactions, especially with drugs prolonging the QT interval.
- Monitor LFTs, especially in patients with pre-existing liver conditions or on prolonged therapy.
- Consider renal dose adjustments for patients with impaired kidney function.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Complete the full course of treatment, even if symptoms improve, to prevent recurrence.
- Report any unusual or severe side effects, especially liver problems (yellowing skin/eyes, dark urine, severe nausea), to your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Fluconazole may cause dizziness or seizures in some individuals. If you experience these side effects, avoid driving or operating machinery.
Lifestyle Advice
- Maintain good hygiene to help prevent fungal infections.
- Avoid irritants that might worsen local fungal conditions.
- Limit alcohol consumption, especially if you have concerns about liver health.
