Overview of the medicine
Lumona 4 mg Oral Granules contain Montelukast, a leukotriene receptor antagonist, used for the treatment of asthma and allergic rhinitis in children aged 6 months to 5 years.
Uses & Indications
Dosage
Adults
Adults and adolescents 15 years and older: 10 mg once daily, in the evening. (Note: This is for 10mg tablets, for 4mg granules, typically for younger children).
Elderly
No dosage adjustment necessary for the elderly.
Renal_impairment
No dosage adjustment necessary for patients with renal impairment.
Children_2_5_years
4 mg once daily, in the evening.
Children_6_months_2_years
4 mg once daily, in the evening.
How to Take
Take orally, once daily in the evening, with or without food. The granules can be administered directly into the mouth, dissolved in a teaspoon of cold or room temperature soft food (e.g., applesauce, ice cream, carrots, rice), or mixed with breast milk or formula. Do not open the sachet until ready to use. Do not store mixture for future use.
Mechanism of Action
Montelukast selectively blocks the CysLT1 receptor, thereby inhibiting the physiological actions of leukotriene D4 (LTD4) and other cysteinyl leukotrienes. Cysteinyl leukotrienes are potent inflammatory eicosanoids released from various cells, including mast cells and eosinophils, involved in the pathophysiology of asthma and allergic rhinitis.

Within one day for asthma and allergic rhinitis.
Excretion
Primarily excreted in the feces via bile (approximately 86%), with less than 0.2% excreted in urine.
Half life
Plasma half-life ranges from 2.7 to 5.5 hours in healthy adults.
Absorption
Rapidly absorbed after oral administration. Mean oral bioavailability is 64%. Peak plasma concentrations achieved in 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C8, and to a lesser extent by CYP3A4 and CYP2C9.
