Description
Overview of the medicine
Marcus 10 mg Tablet contains Aripiprazole, an atypical antipsychotic medication used to treat schizophrenia, bipolar I disorder, and as an adjunctive treatment for major depressive disorder. It helps to restore the balance of certain natural substances in the brain (neurotransmitters).
Uses & Indications
Dosage
Adults
Schizophrenia: Initial 10-15 mg once daily, target 10-15 mg/day. Max 30 mg/day. Bipolar I Disorder (acute mania/mixed episodes): Initial 15 mg once daily as monotherapy or 10-15 mg adjunctive. Max 30 mg/day. Major Depressive Disorder (adjunctive): Initial 2-5 mg once daily, target 5-15 mg/day. Max 15 mg/day.
Elderly
Generally, no dosage adjustment is required, but caution is advised. Start with a lower dose and titrate slowly.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Marcus tablets can be taken with or without food, usually once daily. Swallow the tablet whole with water.
Mechanism of Action
Aripiprazole is thought to work through a combination of partial agonism at D2 and 5-HT1A receptors, and antagonism at 5-HT2A receptors. This unique mechanism helps to stabilize dopamine and serotonin systems in the brain.
Pharmacokinetics
Onset
Clinical effects can be observed within a few days to weeks, full effect may take longer.
Excretion
Mainly through feces (55%) and urine (25%).
Half life
Approximately 75 hours for aripiprazole and 94 hours for its active metabolite dehydro-aripiprazole.
Absorption
Well absorbed orally, peak plasma concentrations typically occur within 3-5 hours.
Metabolism
Primarily hepatic, via CYP2D6 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to aripiprazole or any component of the formulation.
Drug Interactions
Antihypertensive agents
May potentiate orthostatic hypotension.
CYP3A4 inducers (e.g., carbamazepine)
May decrease aripiprazole levels; dose increase of aripiprazole may be necessary.
Other CNS depressants (e.g., alcohol, benzodiazepines)
May potentiate sedative effects.
CYP3A4 inhibitors (e.g., ketoconazole, fluoxetine, paroxetine)
May increase aripiprazole levels; dose reduction of aripiprazole may be necessary.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include somnolence, vomiting, tremor, and tachycardia. Management involves symptomatic and supportive treatment, maintaining airway, oxygenation, and ventilation. Activated charcoal may be useful.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if aripiprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
