Description
Overview of the medicine
Marzev 125 mg Tablet is a medication primarily used for the management of hypertension (high blood pressure) and certain cardiovascular conditions. It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Initial dose 125 mg once daily. May be increased to 250 mg once daily after 7-14 days if needed for blood pressure control. Max dose: 500 mg daily.
Elderly
Consider lower initial dose (e.g., 62.5 mg once daily) and titrate slowly due to potential for increased sensitivity and reduced drug clearance.
Renal_impairment
No dose adjustment is typically required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally with or without food, preferably at the same time each day. Swallow the tablet whole; do not crush, chew, or break it.
Mechanism of Action
Marzev acts as a calcium channel blocker, specifically targeting L-type calcium channels. By inhibiting the influx of calcium ions into vascular smooth muscle cells and myocardial cells, it causes peripheral vasodilation, reduces systemic vascular resistance, and lowers blood pressure. It may also have anti-anginal effects by improving myocardial oxygen supply.
Pharmacokinetics
Onset
Onset of action within 30-60 minutes, peak plasma concentrations typically reached in 2-4 hours.
Excretion
Primarily excreted in urine (approx. 60-70%) as metabolites, with a smaller portion excreted in feces.
Half life
Terminal elimination half-life is approximately 10-15 hours, allowing for once-daily dosing.
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract; bioavailability is approximately 60-70% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily via cytochrome P450 (CYP3A4) enzymes, into inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Marzev or any component of the formulation
- Severe hypotension (systolic blood pressure <90 mmHg)
- Cardiogenic shock
- Severe aortic stenosis
Drug Interactions
Other Antihypertensives
Additive hypotensive effects may occur when co-administered with other blood pressure-lowering agents.
CYP3A4 Inducers (e.g., Rifampin, Phenytoin)
May decrease Marzev plasma concentrations, potentially reducing its therapeutic effect. Dose adjustment may be necessary.
CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
May increase Marzev plasma concentrations, leading to increased risk of adverse effects. Monitor blood pressure closely.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe hypotension, bradycardia, dizziness, and collapse. Treatment is supportive, including intravenous fluids, vasopressors, and atropine if bradycardia is severe. Activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Marzev is excreted in human milk; caution should be exercised when administered to a nursing woman.
