Description
Overview of the medicine
Marzev 25 mg Tablet is a diuretic medicine used to treat high blood pressure (hypertension) and edema caused by various conditions such as heart failure, kidney disease, or liver disease. It helps the body get rid of excess water and salt.
Uses & Indications
Dosage
Adults
For hypertension: 25 mg once daily, preferably in the morning. May be increased to 50 mg if necessary. For edema: 25-50 mg once or twice daily, or intermittently.
Elderly
Initiate with a lower dose (e.g., 12.5 mg) and titrate based on response and tolerability.
Renal_impairment
Use with caution. Not effective in severe renal impairment (creatinine clearance <30 mL/min). Lower doses may be required.
How to Take
Take Marzev 25 mg Tablet orally with or without food, preferably in the morning to avoid nocturnal diuresis. Swallow the tablet whole with water.
Mechanism of Action
Marzev-thiazide, a thiazide-like diuretic, acts primarily in the distal convoluted tubule of the kidney. It inhibits the sodium-chloride symporter, leading to increased excretion of sodium, chloride, and water, thereby reducing blood volume and blood pressure.
Pharmacokinetics
Onset
Diuretic effect: 2 hours; Antihypertensive effect: 2 hours.
Excretion
Primarily excreted unchanged by the kidneys (approximately 50-70% within 24 hours).
Half life
Approximately 6-15 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is moderate (50-60%).
Metabolism
Minimal hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to Marzev-thiazide or other sulfonamide-derived drugs
- Anuria (absence of urine production)
- Severe renal impairment
- Severe hepatic impairment
Drug Interactions
Lithium
Thiazides reduce renal clearance of lithium, increasing the risk of lithium toxicity.
Corticosteroids
Increased risk of electrolyte imbalance, particularly hypokalemia.
Antidiabetic drugs
Thiazides may decrease the glucose-lowering effect of antidiabetic agents, requiring dose adjustments.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the diuretic and antihypertensive effects of thiazides.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include electrolyte depletion (hypokalemia, hypochloremia, hyponatremia), dehydration, hypotension, and dizziness. Management involves symptomatic and supportive treatment, including correction of fluid and electrolyte imbalance. Gastric lavage or activated charcoal may be considered in recent ingestions.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed, as it crosses the placental barrier. Not recommended during lactation as it is excreted in breast milk and may suppress lactation. Consult a doctor.
