Description
Overview of the medicine
Maxical-M is a nutritional supplement combining Calcium, Vitamin D3, Magnesium, and Zinc, primarily used for maintaining bone health, preventing osteoporosis, and addressing deficiencies of these vital minerals and vitamins.
Uses & Indications
Dosage
Adults
1-2 tablets daily, preferably with or after meals, or as directed by a physician.
Elderly
1-2 tablets daily, or as directed by a physician.
Renal_impairment
Use with caution and monitor calcium levels. Dosage adjustment may be required.
How to Take
Swallow the tablet whole with a glass of water. It can be taken with or without food, but taking it with food may improve absorption and reduce gastrointestinal upset.
Mechanism of Action
Calcium is essential for bone formation and maintenance. Vitamin D3 helps in the absorption of calcium and phosphorus from the gut. Magnesium contributes to bone mineral density and regulates calcium metabolism. Zinc plays a role in bone matrix formation and bone mineralization.
Pharmacokinetics
Onset
Gradual; significant effects on bone density typically seen after weeks to months of continuous use.
Excretion
Primarily renal excretion for calcium, magnesium, and zinc; some biliary and fecal excretion for vitamin D3 metabolites.
Half life
Vitamin D3 has a relatively long half-life (several weeks), while calcium, magnesium, and zinc ions are rapidly cleared or incorporated.
Absorption
Calcium and Vitamin D3 are primarily absorbed in the small intestine. Magnesium and Zinc are also absorbed from the gastrointestinal tract.
Metabolism
Vitamin D3 is hydroxylated in the liver and kidneys to its active form. Calcium, magnesium, and zinc are primarily utilized as ions and are not extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to any component of the formulation
- •Hypercalcemia (high levels of calcium in the blood)
- •Hypervitaminosis D (excessive levels of Vitamin D)
- •Severe renal impairment
- •Kidney stones (nephrolithiasis) or history of recurrent stone formation
Drug Interactions
Bisphosphonates
Reduced absorption if taken concurrently. Separate administration by at least 30 minutes to 2 hours.
Thiazide Diuretics
May reduce urinary calcium excretion, potentially leading to hypercalcemia.
Tetracyclines & Fluoroquinolones
Calcium, magnesium, and zinc can chelate with these antibiotics, reducing their absorption. Administer at least 2-4 hours apart.
Cardiac Glycosides (e.g., Digoxin)
Hypercalcemia can potentiate the effects of cardiac glycosides, increasing the risk of toxicity.
Thyroid Hormones (e.g., Levothyroxine)
Calcium can interfere with the absorption of thyroid hormones. Administer at least 4 hours apart.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, abdominal pain, muscle weakness, confusion), hypercalciuria, and in severe cases, renal impairment, cardiac arrhythmias, and coma. Treatment involves discontinuation of the supplement, rehydration, and supportive care. Consult a doctor immediately.
Pregnancy & Lactation
Maxical-M can be used during pregnancy and lactation under medical supervision to meet increased mineral and vitamin requirements, but the dosage should be carefully monitored to avoid hypercalcemia or hypervitaminosis D.
