Maxidim 500 mg Injection contains Cefepime, a fourth-generation cephalosporin antibiotic used to treat various moderate to severe bacterial infections. It is effective against a broad spectrum of Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
Usual dose is 1-2 grams intravenously (IV) or intramuscularly (IM) every 8-12 hours. For severe infections, 2 grams IV every 8 hours.
Elderly
Similar to adult dosage, but dose adjustment is necessary based on renal function due to age-related decline in kidney function.
Renal_impairment
Dosage must be adjusted based on creatinine clearance (CrCl) to prevent accumulation and potential neurotoxicity. Lower doses or extended dosing intervals are typically required.
How to Take
Maxidim 500 mg Injection is administered intravenously (IV) via slow infusion (over 30-60 minutes) or intramuscularly (IM) via deep injection. The powder must be reconstituted with an appropriate diluent (e.g., sterile water for injection) before use.
Mechanism of Action
Cefepime is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the transpeptidation step of peptidoglycan synthesis. This leads to the disruption of the cell wall, ultimately resulting in bacterial cell lysis and death.

Onset
Rapid onset of action, typically within minutes after IV administration.
Excretion
Primarily excreted unchanged by the kidneys, predominantly via glomerular filtration. About 85% of the administered dose is recovered in the urine.
Half life
Approximately 2 hours in individuals with normal renal function.
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are achieved within 0.5 to 1.5 hours after IM injection.
Metabolism
Cefepime is minimally metabolized in the liver; about 10-20% is hydrolyzed to an inactive metabolite (N-methylpyrrolidine).
Side Effects
Contraindications
- •Hypersensitivity to Cefepime, other cephalosporins, penicillins, or any component of the formulation.
- •History of immediate and severe hypersensitivity reactions to any beta-lactam antibiotic.
Drug Interactions
Aminoglycosides
Concurrent administration may increase the risk of nephrotoxicity and ototoxicity.
Diuretics (e.g., Furosemide)
High doses of cephalosporins with potent diuretics may increase the risk of renal toxicity.
Oral Anticoagulants (e.g., Warfarin)
May potentiate the anticoagulant effect, increasing the risk of bleeding. Close INR monitoring is advised.
Storage
Store unopened vials at room temperature (below 25°C), protected from light. After reconstitution, the solution should be used immediately or stored under refrigeration (2-8°C) for a limited period as per manufacturer guidelines.
Overdose
In cases of severe overdose, symptoms may include encephalopathy, seizures, myoclonus, and neuromuscular excitability. Management should be supportive. Hemodialysis is effective in removing cefepime from the body and may be beneficial in severe overdose with renal impairment.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, use during pregnancy only if clearly needed. Cefepime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing woman.
