Description
Overview of the medicine
Maximox 400 mg Tablet is an antibiotic belonging to the penicillin group. It is used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
The usual adult dose is 250 mg to 500 mg every 8 hours, or 500 mg to 875 mg every 12 hours, depending on the severity and type of infection. For Maximox 400 mg tablet: 400 mg every 8-12 hours.
Elderly
Similar to adult dosage, but renal function should be assessed and dosage adjusted if necessary.
Renal_impairment
Dosage reduction is required for patients with severe renal impairment (creatinine clearance <30 mL/min). Consult a doctor for specific dosage adjustments.
How to Take
Maximox tablet can be taken orally with or without food. It is recommended to take it at regular intervals to maintain a consistent level of the antibiotic in the body. Do not chew or crush the tablet.
Mechanism of Action
Amoxicillin works by inhibiting the synthesis of bacterial cell walls, leading to bacterial lysis and death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which are essential for the synthesis of peptidoglycan.
Pharmacokinetics
Onset
Rapid, typically within 1-2 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (50-70%) within 6-8 hours by renal tubular secretion and glomerular filtration.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration. Bioavailability is approximately 70-90%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Partially metabolized (10-25%) in the liver to inactive penicilloic acid.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin, other penicillins, or any component of the formulation.
- History of severe immediate hypersensitivity reaction (e.g., anaphylaxis) to another beta-lactam antibiotic (e.g., cephalosporin, carbapenem, monobactam).
Drug Interactions
Probenecid
Increases and prolongs amoxicillin blood levels by reducing renal tubular secretion.
Allopurinol
Increased incidence of rash, especially in patients with hyperuricemia.
Methotrexate
Amoxicillin may reduce renal clearance of methotrexate, potentially increasing toxicity.
Oral contraceptives
May reduce the efficacy of oral contraceptives; advise using alternative birth control methods.
Warfarin and other anticoagulants
May prolong prothrombin time; monitor INR closely.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and disturbances of fluid and electrolyte balance. In cases of massive overdose, renal failure and convulsions may occur. Management involves symptomatic and supportive treatment, including gastric lavage or emesis if recent ingestion. Hemodialysis can be used to remove amoxicillin from the circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Amoxicillin is generally considered safe during pregnancy but should be used only if clearly needed. It is excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor for diarrhea, thrush, or rash in the infant.
