Overview of the medicine
Maxineb 5 mg Tablet is an antidepressant belonging to the Selective Serotonin Reuptake Inhibitor (SSRI) class. It is used to treat major depressive disorder, anxiety disorders, and certain other mood conditions by helping to restore the balance of a natural substance (serotonin) in the brain.
Uses & Indications
Dosage
Adults
Initial dose: 5 mg once daily, preferably in the morning. May be increased to 10-20 mg/day after several weeks based on response and tolerability.
Elderly
Initial dose: 5 mg once daily. Max 10 mg/day due to reduced metabolism and increased sensitivity.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe impairment; consider lower starting dose.
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole with water; do not crush or chew.
Mechanism of Action
Maxineboxine selectively inhibits the reuptake of serotonin (5-HT) in the central nervous system, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to enhanced serotonergic neurotransmission, which is believed to be responsible for its antidepressant and anxiolytic effects.

Onset
Initial effects (e.g., sleep, appetite) may be noticed within a few days, but full antidepressant effects typically take 2-4 weeks.
Excretion
Excreted mainly through the urine (approximately 60%) and feces (approximately 30%) as metabolites.
Half life
Approximately 18-24 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, peak plasma concentration reached within 4-6 hours. Food does not significantly affect absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes, into inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to maxineboxine or any component of the tablet.
- •Concurrent use of Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment.
Drug Interactions
MAOIs
Concomitant use is contraindicated due to risk of serotonin syndrome (e.g., fever, rigidity, mental status changes). Allow 14 days between stopping MAOI and starting Maxineb, and vice versa.
NSAIDs/Warfarin
Increased risk of bleeding due to potential antiplatelet effect of SSRIs. Monitor INR for warfarin users.
Serotonergic Drugs
Increased risk of serotonin syndrome with other serotonergic agents (e.g., Triptans, Tricyclic Antidepressants, Fentanyl, Lithium, Tramadol, St. John's Wort). Use with caution and monitor for symptoms.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, dizziness, tremor, tachycardia, and rarely, seizures or serotonin syndrome. Treatment is symptomatic and supportive. Maintain an open airway, monitor cardiac and vital signs. Activated charcoal may be considered soon after ingestion.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk; caution advised during lactation due to potential adverse effects on the infant.
