Description
Overview of the medicine
Maxpain is a combination medicine containing Paracetamol and Caffeine, used for the relief of mild to moderate pain and fever. Paracetamol provides pain relief and reduces fever, while Caffeine enhances the analgesic effect and provides a mild stimulant action.
Uses & Indications
Dosage
Adults
1-2 tablets every 4-6 hours as needed. Do not exceed 8 tablets (4000 mg paracetamol) in 24 hours.
Elderly
No special dosage adjustment is typically required, but caution is advised in elderly patients with impaired renal or hepatic function.
Renal_impairment
Dose interval may need to be extended in patients with severe renal impairment (CrCl <10 ml/min).
How to Take
Oral administration. Tablets can be taken with or without food. Swallow with water.
Mechanism of Action
Paracetamol works by inhibiting prostaglandin synthesis, primarily in the central nervous system, thereby exerting analgesic and antipyretic effects. Caffeine acts as a central nervous system stimulant and is believed to enhance the pain-relieving effects of paracetamol.
Pharmacokinetics
Onset
Onset of analgesic action typically occurs within 30 minutes.
Excretion
Both components and their metabolites are primarily excreted via the kidneys in urine.
Half life
Paracetamol has a plasma half-life of 1-4 hours. Caffeine has a plasma half-life of approximately 3-7 hours.
Absorption
Both paracetamol and caffeine are rapidly and almost completely absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are usually reached within 30-60 minutes for paracetamol.
Metabolism
Paracetamol is primarily metabolized in the liver (glucuronidation and sulfation pathways). Caffeine is extensively metabolized in the liver by the cytochrome P450 system.
Side Effects
Contraindications
- Hypersensitivity to paracetamol, caffeine, or any other component of the formulation.
- Severe hepatic impairment or active liver disease.
- Severe hypertension or severe coronary artery disease (due to caffeine).
Drug Interactions
0
Warfarin and other coumarin anticoagulants: Increased risk of bleeding with prolonged, high-dose paracetamol use.
1
Alcohol: Increased risk of liver damage with paracetamol.
2
Certain antibiotics (e.g., ciprofloxacin, enoxacin): May reduce caffeine clearance, leading to increased caffeine levels.
3
Monoamine Oxidase Inhibitors (MAOIs): Concurrent use with caffeine may lead to hypertensive crisis.
4
Oral contraceptives: May reduce caffeine clearance.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of paracetamol overdose include nausea, vomiting, abdominal pain, and liver damage, which can be severe. Caffeine overdose symptoms may include anxiety, restlessness, insomnia, tremor, tachycardia, and potentially seizures. In case of overdose, seek immediate medical attention. Treatment involves gastric decontamination and administration of N-acetylcysteine for paracetamol overdose. Symptomatic and supportive care for caffeine overdose.
Pregnancy & Lactation
Paracetamol is generally considered safe during pregnancy when used at recommended doses. Caffeine crosses the placenta and enters breast milk; therefore, consult a doctor before use during pregnancy or lactation. High doses of caffeine should be avoided.

Lab Monitoring
- Liver function tests (LFTs) may be monitored in patients taking chronic high doses or those at risk of hepatotoxicity.
- Renal function may be assessed in patients with pre-existing renal impairment.
Doctor Notes
- Advise patients not to exceed the maximum daily dose to prevent liver toxicity.
- Educate patients about the caffeine content and the need to limit other caffeine sources.
- Caution in patients with pre-existing liver disease or severe cardiovascular conditions.
