Description
Overview of the medicine
Mebantrin is an anthelmintic medication used to treat a variety of parasitic worm infections, including roundworm, whipworm, pinworm, and hookworm. It works by interfering with the worm's ability to absorb glucose, leading to their starvation and death. The chewable tablet form is convenient for administration, especially in children.
Uses & Indications
Dosage
Adults
For pinworm: 100 mg chewable tablet as a single dose. May be repeated after 2-4 weeks if necessary. For roundworm, whipworm, and hookworm: 100 mg chewable tablet twice daily (morning and evening) for 3 consecutive days.
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised in those with significant hepatic impairment.
Renal_impairment
No dosage adjustment is considered necessary for patients with renal impairment, as systemic absorption is minimal and renal excretion is not the primary route.
How to Take
The tablet should be thoroughly chewed before swallowing. It can be taken with or without food. For young children or individuals with difficulty chewing, the tablet can be crushed and mixed with a small amount of food.
Mechanism of Action
Mebantrin selectively and irreversibly blocks glucose uptake and other nutrients in susceptible intestinal helminths. This leads to the depletion of glycogen stores, reduced ATP formation, and ultimately the immobilization and death of the parasites. It achieves this by binding to beta-tubulin, disrupting microtubule formation in the worm's intestinal cells.
Pharmacokinetics
Onset
Clinical effects (worm expulsion) typically observed within 1-3 days, depending on the type of infection and dosage regimen.
Excretion
Primarily excreted in the feces as unchanged drug and metabolites. A small amount (<2%) is excreted in the urine.
Half life
Approximately 2.5 to 5.5 hours for the parent drug.
Absorption
Poorly absorbed from the gastrointestinal tract (less than 10% bioavailability). Absorption is slightly increased with a fatty meal.
Metabolism
Extensively metabolized in the liver, primarily to its hydroxylated metabolite (e.g., hydroxymebantrin), which has little to no anthelmintic activity.
Side Effects
Contraindications

Drug Interactions
Cimetidine
May increase plasma concentrations of mebantrin by inhibiting its metabolism, potentially leading to increased side effects.
Metronidazole
Concomitant use with metronidazole has been associated with an increased risk of Stevens-Johnson syndrome/toxic epidermal necrolysis; concurrent use should generally be avoided.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include abdominal cramps, nausea, vomiting, and diarrhea. In cases of overdose, treatment should be symptomatic and supportive. Gastric lavage may be performed, and activated charcoal can be administered to reduce absorption.
Pregnancy & Lactation
Pregnancy Category C. Mebantrin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether mebantrin is excreted in human milk. Caution should be exercised when administered to a nursing woman.
