Description
Overview of the medicine
Mebrex 100 mg Suspension contains Mebendazole, an anthelmintic medication used to treat various types of worm infestations such as pinworm, roundworm, whipworm, and hookworm. It works by inhibiting the worms' ability to absorb sugar, leading to their starvation and death.
Uses & Indications
Dosage
Adults
Pinworm: 100 mg single dose; repeat after 2-4 weeks if necessary. Other common worm infections (roundworm, hookworm, whipworm): 100 mg twice daily for 3 consecutive days.
Elderly
No specific dose adjustment is generally required for elderly patients. Dosage as per adults.
Renal_impairment
No dose adjustment is necessary in patients with renal impairment due to minimal systemic absorption.
How to Take
Take orally with or without food. The suspension can be taken directly or mixed with a small amount of liquid or food. For young children, it can be mixed with food to ensure intake.
Mechanism of Action
Mebendazole selectively binds to beta-tubulin in the intestinal cells of susceptible helminths, leading to the inhibition of microtubule polymerization. This impairs the uptake of glucose and other nutrients, resulting in the depletion of glycogen stores, reduced ATP synthesis, and ultimately, the immobilization and death of the worms.
Pharmacokinetics
Onset
Therapeutic effects are typically observed within days of treatment, depending on the worm type and lifecycle.
Excretion
Mainly excreted as metabolites and unchanged drug in feces. A small percentage is excreted in urine.
Half life
Plasma half-life is approximately 2.5 to 5.5 hours. Metabolites have longer half-lives.
Absorption
Poorly absorbed from the gastrointestinal tract (typically <10%). Absorption may increase with fatty meals.
Metabolism
Extensively metabolized in the liver (primarily via CYP450 enzymes) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to mebendazole or any component of the formulation.
- Children under 2 years of age (due to reports of convulsions in this age group).
Drug Interactions
Cimetidine
May inhibit the metabolism of mebendazole, leading to increased plasma concentrations.
Phenytoin, Carbamazepine
May decrease the plasma concentrations of mebendazole due to induction of hepatic enzymes.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
In cases of accidental overdose, gastrointestinal symptoms such as abdominal cramps, nausea, vomiting, and diarrhea may occur. Treatment is symptomatic and supportive, including gastric lavage or induced emesis.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts are excreted in breast milk; caution should be exercised when administered to a nursing mother.
