Description
Overview of the medicine
Melafresh cream is a combination medication used to treat moderate to severe melasma on the face. It helps in reducing dark spots, hyperpigmentation, and blemishes by exfoliating the skin and inhibiting melanin production.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected areas once daily at night, typically for up to 8 weeks. Discontinue after 8 weeks or when melasma resolves, whichever occurs first.
Elderly
No specific dose adjustment required, use with caution.
Renal_impairment
No specific dose adjustment required, as systemic absorption is minimal.
How to Take
For external use only. Apply a thin film of cream evenly to the hyperpigmented areas of the face, including about 0.5 cm of normal-appearing skin surrounding the lesions. Do not apply to eyes, mouth, nose, or open wounds. Wash hands before and after application.
Mechanism of Action
Hydroquinone works by inhibiting tyrosinase, an enzyme crucial for melanin production. Tretinoin (a retinoid) promotes cell turnover and exfoliation, helping to shed pigmented cells. Mometasone Furoate (a corticosteroid) reduces inflammation and irritation, allowing the other two ingredients to work effectively while minimizing side effects.
Pharmacokinetics
Onset
Visible improvement typically within 4-6 weeks.
Excretion
Primarily excreted locally or via desquamation; minimal systemic excretion.
Half life
Not clinically significant due to minimal systemic absorption.
Absorption
Minimal systemic absorption after topical application. Primarily local action.
Metabolism
Primarily metabolized locally in the skin.
Side Effects
Contraindications
- •Hypersensitivity to any of the ingredients.
- •Pregnancy and lactation.
- •Children under 12 years of age.
- •Eczema, open wounds, irritated or sunburned skin.
- •Acute skin infections.
Drug Interactions
Other topical retinoids or irritants
Increased risk of irritation. Avoid concurrent use.
Sulfonamides, resorcinol, salicylic acid
May cause darkening of the skin (ochronosis) with hydroquinone. Avoid concurrent use.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Excessive or prolonged application may lead to increased side effects such as severe irritation, peeling, redness, and sensitivity. Systemic absorption leading to toxicity is unlikely but possible with very large, prolonged application.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation due to potential risks to the fetus/infant, particularly with Tretinoin and Mometasone Furoate. A negative pregnancy test is recommended before starting treatment, and effective contraception should be used during treatment.
