Description
Overview of the medicine
Betahistine is used to treat symptoms of Meniere's disease such as dizziness, ringing in the ears (tinnitus), hearing loss, and nausea. It improves blood flow to the inner ear.
Uses & Indications
Dosage
Adults
Typically 8-16 mg three times daily, preferably with meals. Dosage may be adjusted based on clinical response.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No specific dosage adjustment recommended, use with caution.
How to Take
For oral administration. Tablets should be taken with or after meals to avoid gastric irritation.
Mechanism of Action
Betahistine acts as a weak histamine H1 receptor agonist and a strong histamine H3 receptor antagonist. It enhances microcirculation in the inner ear, reduces endolymphatic pressure, and may modulate neuronal activity in the vestibular nuclei.
Pharmacokinetics
Onset
Clinical effects may take several days to weeks to manifest.
Excretion
Excreted predominantly in the urine as metabolites, mainly 2-PAA.
Half life
Approximately 3-4 hours (for 2-pyridylacetic acid, the active metabolite).
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1 hour.
Metabolism
Extensively metabolized to 2-pyridylacetic acid (2-PAA), an inactive metabolite, via oxidative deamination.
Side Effects
Contraindications
- •Hypersensitivity to betahistine or any component of the tablet
- •Pheochromocytoma (a rare tumor of the adrenal gland)
Drug Interactions
Antihistamines
May reduce the therapeutic effect of betahistine.
MAO Inhibitors
May increase exposure to betahistine, caution advised.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, abdominal pain, drowsiness, and convulsions at very high doses. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed and after consulting a doctor. Not recommended during breastfeeding due to lack of data on excretion into human milk.
