Overview of the medicine
Menaril 8 mg Tablet is a medication used for the management of symptoms associated with Ménière's disease, such as vertigo (dizziness), tinnitus (ringing in the ears), and hearing loss.
Uses & Indications
Dosage
Adults
Usually 8-16 mg three times daily, preferably with meals. Dosage may be adjusted based on individual response, up to 48 mg/day.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required for patients with renal impairment.
How to Take
Take tablets orally, preferably with meals to reduce gastrointestinal discomfort.
Mechanism of Action
Betahistine acts as a weak histamine H1-receptor agonist and a potent histamine H3-receptor antagonist. It is thought to improve microcirculation in the inner ear by relaxing pre-capillary sphincters, leading to a reduction in endolymphatic pressure and improved cochlear blood flow. It also suppresses vestibular nuclei activity.

Onset
Symptomatic relief may be observed within hours to days, but optimal effects may take several weeks.
Excretion
Excreted mainly in the urine as its metabolites, primarily 2-pyridylacetic acid.
Half life
Approximately 3-4 hours (for its main metabolite, 2-pyridylacetic acid).
Absorption
Rapidly and almost completely absorbed after oral administration. Peak plasma concentration is reached within 1 hour.
Metabolism
Extensively metabolized in the liver via oxidative deamination to 2-pyridylacetic acid, which is largely inactive.
Side Effects
Contraindications
- •Known hypersensitivity to Betahistine or any component of the formulation
- •Pheochromocytoma (a tumor of the adrenal gland)
- •Bronchial asthma
- •Peptic ulcer (use with caution)
Drug Interactions
Antihistamines
May reduce the efficacy of betahistine due to antagonistic effects on histamine receptors. Concurrent use should be avoided.
MAOIs (Monoamine Oxidase Inhibitors)
MAOIs may inhibit the metabolism of betahistine, leading to increased exposure. Caution is advised.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, abdominal pain, drowsiness, and convulsions at very high doses. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown teratogenic effects, but adequate and well-controlled studies in pregnant women are lacking. Should be used during pregnancy only if clearly needed. Avoid during lactation as it is unknown if betahistine is excreted in human milk.
