Overview of the medicine
Merocar 1 gm Injection contains Meropenem, a broad-spectrum carbapenem antibiotic used to treat serious bacterial infections such as pneumonia, complicated intra-abdominal infections, meningitis, and complicated skin and skin structure infections.
Uses & Indications
Dosage
Adults
Typical dose is 500 mg to 1 gm IV every 8 hours, or 2 gm IV every 8 hours for very severe infections. Dosage should be adjusted based on infection type, severity, and susceptibility of the pathogen.
Elderly
Dosage adjustment is usually not required in elderly patients with normal renal function. For those with impaired renal function, dosage should be adjusted as for renal impairment.
Renal_impairment
Dose reduction is necessary based on creatinine clearance (e.g., for CrCl 26-50 mL/min, 500 mg or 1 gm every 12 hours; for CrCl 10-25 mL/min, 250 mg or 500 mg every 12 hours).
How to Take
Merocar 1 gm Injection is administered intravenously (IV). It can be given as an IV bolus injection over 3-5 minutes (for doses up to 1 gm) or as an IV infusion over 15-30 minutes.
Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It is resistant to hydrolysis by most beta-lactamases.

Primarily renal, with approximately 70% of the administered dose excreted unchanged in urine within 12 hours.

Side Effects
Contraindications

Drug Interactions
Probenecid
Probenecid competes with meropenem for active tubular secretion, resulting in increased plasma concentrations and a prolonged half-life of meropenem. Co-administration is not recommended unless clinically justified.
Valproic Acid
Co-administration with meropenem can significantly decrease valproic acid plasma concentrations, potentially leading to subtherapeutic levels and increased risk of seizures. Concurrent use should generally be avoided.
Storage
Store the dry powder at room temperature below 25-30°C. Protect from light. Do not freeze. Reconstituted solutions should be used immediately or stored as per specific instructions (e.g., refrigerated for a limited time).
Overdose
In case of overdose, meropenem can be removed by hemodialysis. Treatment should be symptomatic and supportive. Monitor vital signs and provide general supportive measures.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Meropenem is excreted in human milk in very low concentrations; caution should be exercised when administered to a nursing woman.
