Merocar 500 mg Injection contains Meropenem, a broad-spectrum carbapenem antibiotic used to treat a wide range of severe bacterial infections, including those resistant to other antibiotics. It is administered intravenously.
Uses & Indications
Dosage
Adults
Usually 500 mg to 1 g IV every 8 hours, depending on the type and severity of infection. Administer over 15-30 minutes by IV infusion or over 3-5 minutes by IV bolus.
Elderly
Dosage adjustment may be necessary based on renal function. Generally, the lower end of the adult dose range is considered.
Renal_impairment
Significant dosage reduction is required based on creatinine clearance (e.g., for CrCl 26-50 mL/min, 500 mg every 12 hours; for CrCl 10-25 mL/min, 250 mg every 12 hours).
How to Take
For intravenous use only. Merocar 500 mg Injection can be administered as an intravenous bolus over 3-5 minutes or as an intravenous infusion over 15-30 minutes. Reconstitute the powder for injection with sterile water for injection or other compatible diluents.
Mechanism of Action
Meropenem is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This leads to the disruption of the cell wall and ultimately bacterial cell death.

Side Effects
Contraindications
- •Hypersensitivity to meropenem or any other carbapenem antibiotics.
- •Severe hypersensitivity (e.g., anaphylactic reaction, severe skin reaction) to any other beta-lactam antibacterial agent (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Probenecid competes with meropenem for active tubular secretion, resulting in increased plasma concentrations and prolonged half-life of meropenem.
Valproic Acid
Co-administration with meropenem may decrease valproic acid plasma concentrations, leading to subtherapeutic levels and increased risk of seizures.
Storage
Store dry powder for injection below 25-30°C, protected from light. Do not freeze. Store reconstituted solution as per manufacturer's instructions.
Overdose
In the event of an overdose, discontinue meropenem and initiate supportive care. Meropenem can be removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Meropenem is excreted in human breast milk; therefore, caution should be exercised when administered to a nursing mother.
