Overview of the medicine
Meroject 1 gm Injection contains Meropenem, which is a broad-spectrum carbapenem antibiotic. It is used to treat serious bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
Usual dose is 0.5 gm to 2 gm intravenously every 8 hours, depending on the type and severity of infection. For meningitis, 2 gm every 8 hours.
Elderly
Dosage adjustment may be necessary based on renal function. No specific dosage adjustment required solely for age.
Renal_impairment
Dosage should be reduced based on creatinine clearance (CrCl). For CrCl 26-50 mL/min, administer full dose every 12 hours; for CrCl 10-25 mL/min, administer half dose every 12 hours; for CrCl <10 mL/min, administer half dose every 24 hours.
How to Take
Meroject 1 gm Injection should be administered by intravenous infusion over 15 to 30 minutes, or by intravenous bolus injection (500 mg to 1 gm) over 3 to 5 minutes.
Mechanism of Action
Meropenem is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It penetrates the bacterial cell and binds to penicillin-binding proteins (PBPs), leading to inhibition of peptidoglycan synthesis and ultimately bacterial cell lysis and death.

Excretion
Approximately 70% of the administered dose is recovered unchanged in the urine within 12 hours. Renal elimination is the primary route.

Approximately 1 hour (in adults with normal renal function)
Absorption
Rapid and complete absorption after intravenous administration. Peak plasma concentrations are achieved shortly after the end of infusion.
Metabolism
Minimal hepatic metabolism. Primarily hydrolyzed by the ring-opening of the beta-lactam moiety to an inactive metabolite, though it is stable to dehydropeptidase-I.
Side Effects
Contraindications
- Known hypersensitivity to meropenem, any other carbapenem antibiotics, or any component of the formulation.
- History of severe hypersensitivity reaction to any other beta-lactam antibacterial agent (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Probenecid competes with meropenem for active tubular secretion, leading to increased plasma concentrations and prolonged half-life of meropenem. Co-administration with probenecid is not recommended.
Valproic acid/Divalproex sodium
Carbapenems, including meropenem, may decrease valproic acid plasma concentrations to subtherapeutic levels, leading to loss of seizure control. Concomitant use is generally not recommended.
Storage
Store below 30°C. Protect from light. Do not freeze. Reconstituted solutions should be used immediately or stored according to manufacturer's instructions for a limited time.
Overdose
In the event of an overdose, symptomatic and supportive treatment should be initiated. Meropenem can be removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of fetal harm, but there are no adequate and well-controlled studies in pregnant women. Meropenem is excreted in human milk in very low concentrations. Use during pregnancy and lactation only if clearly needed and the potential benefit justifies the potential risk to the fetus/infant.
