Metformin Extended-Release 750 mg Tablet is an oral medication used to manage type 2 diabetes mellitus. It helps control high blood sugar levels by reducing glucose production in the liver, decreasing glucose absorption from the intestines, and improving insulin sensitivity.
Uses & Indications
Dosage
Adults
Usually 750 mg once daily with the evening meal. Dose may be titrated up by physician based on glucose control and tolerability, up to a maximum of 2250 mg once daily.
Elderly
Initiate at a lower dose and carefully monitor renal function due to increased risk of lactic acidosis.
Renal_impairment
Contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73m²). Dose adjustment required for moderate impairment (eGFR 30-60 mL/min/1.73m²).
How to Take
Swallow the tablet whole with the evening meal. Do not crush, chew, or break the tablet.
Mechanism of Action
Metformin primarily works by decreasing hepatic glucose production (gluconeogenesis), decreasing intestinal absorption of glucose, and by improving insulin sensitivity (increasing peripheral glucose uptake and utilization).

Contraindications

- •Acute or chronic metabolic acidosis, including diabetic ketoacidosis
- •Heart failure requiring pharmacologic treatment
Drug Interactions
Iodinated Contrast Agents
Temporarily discontinue metformin at the time of or prior to an iodinated contrast imaging procedure in patients with an eGFR between 30-60 mL/min/1.73m².
Cimetidine, Ranolazine, Dolutegravir, Dofetilide, Isavuconazole, Vandetanib
May increase metformin plasma concentrations and increase the risk of lactic acidosis.
Diuretics, Corticosteroids, Phenothiazines, Thyroid Products, Estrogens, Oral Contraceptives, Phenytoin, Nicotinic Acid, Sympathomimetics, Calcium Channel Blockers, Isoniazid
May produce hyperglycemia, requiring adjustment of metformin dosage.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Overdose can lead to lactic acidosis. Treatment involves supportive measures and hemodialysis to remove metformin from the body.
Pregnancy & Lactation
Discuss with a doctor if pregnant or breastfeeding. Metformin may be used in pregnancy under medical supervision; insulin is often preferred. Metformin is excreted into breast milk, monitor infant for adverse effects.
