Uses & Indications
Dosage
Adults
Initial dose is typically 500 mg or 750 mg once daily with the evening meal. Dose may be gradually increased by 500 mg weekly to a maximum of 2000 mg once daily. For 1000 mg, it is usually taken once daily with the evening meal.
Elderly
Dose adjustment based on renal function is crucial due to potential age-related decline in kidney function. Start with a lower dose.
Renal_impairment
Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73 m²). Dose adjustment required for moderate impairment; discontinue if eGFR drops below 30.
How to Take
Take with food, usually with the evening meal, to minimize gastrointestinal side effects. Swallow the tablet whole; do not crush, chew, or break it.
Mechanism of Action
It works by decreasing glucose production by the liver, decreasing intestinal absorption of glucose, and improving insulin sensitivity by increasing peripheral glucose uptake and utilization.

Contraindications

Drug Interactions
Alcohol

Cimetidine

Iodinated Contrast Agents
Temporarily discontinue metformin at the time of or prior to iodinated contrast imaging procedures and restart 48 hours later, if renal function is stable, to avoid lactic acidosis.
Diuretics (e.g., Thiazides)
May increase blood glucose levels, potentially requiring metformin dose adjustment.
