Description
Overview of the medicine
An oral antidiabetic medicine used to manage type 2 diabetes mellitus, particularly in overweight individuals, when diet and exercise alone are not enough.
Uses & Indications
Dosage
Adults
Initial dose is usually 500 mg once daily with the evening meal. May be increased by 500 mg weekly up to a maximum of 2000 mg once daily.
Elderly
Dosage should be individualized based on renal function, starting with lower doses to minimize risk of lactic acidosis.
Renal_impairment
Contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73m²). Dose adjustment required for moderate impairment (eGFR 30-60 mL/min/1.73m²).
How to Take
Take orally once daily with the evening meal. Swallow whole, do not crush, chew, or break the tablet to maintain its extended-release properties.
Mechanism of Action
Decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.

Excretion
Excreted unchanged in the urine by glomerular filtration and tubular secretion.

Drug Interactions
Alcohol

Iodinated Contrast Media
Temporarily discontinue metformin before or at the time of an iodinated contrast imaging procedure and for 48 hours afterward. Re-evaluate renal function before restarting.
Carbonic Anhydrase Inhibitors (e.g., topiramate)
May increase risk of lactic acidosis.
Certain Antihypertensives (e.g., ACE inhibitors, ARBs)
Can alter renal function, potentially affecting metformin elimination.
Storage
Store below 30°C (86°F) in a dry place, away from direct light and moisture. Keep out of reach and sight of children.
Overdose
Overdose may lead to lactic acidosis, which is a medical emergency. Management includes hemodialysis for rapid metformin removal and intensive supportive care. Symptoms include malaise, myalgia, respiratory distress, and abdominal pain.
