Description
Overview of the medicine
Methipred 4 mg Tablet contains Methylprednisolone, a synthetic corticosteroid used to treat various inflammatory and autoimmune conditions. It works by reducing inflammation and suppressing the immune system.
Uses & Indications
Dosage
Adults
Initial dosage varies from 4 mg to 48 mg daily, depending on the specific disease being treated. For chronic conditions, maintenance dose should be kept at the lowest effective level.
Elderly
Use with caution, lowest effective dose. Increased risk of side effects.
Renal_impairment
Generally no dose adjustment needed for renal impairment, but caution with fluid retention.
How to Take
Oral. Should be taken with food or milk to minimize gastrointestinal irritation.
Mechanism of Action
Methylprednisolone works by binding to intracellular glucocorticoid receptors. This complex then translocates to the nucleus and modulates gene expression, leading to anti-inflammatory and immunosuppressive effects. It inhibits the release of inflammatory mediators like prostaglandins and leukotrienes.
Pharmacokinetics
Onset
Hours to days, depending on condition and dosage.
Excretion
Excreted mainly in the urine as inactive metabolites.
Half life
Biological half-life is 18-36 hours; plasma half-life is 2-3 hours.
Absorption
Well absorbed orally. Peak plasma concentrations are reached in 1-2 hours.
Metabolism
Primarily metabolized in the liver by CYP3A4 enzymes.
Side Effects
Contraindications
- Systemic fungal infections
- Known hypersensitivity to methylprednisolone or any component of the formulation
- Administration of live or live-attenuated vaccines (when immunosuppressive doses are used)
Drug Interactions
Warfarin
Methylprednisolone can either potentiate or inhibit the anticoagulant effect of warfarin; monitor INR.
Diuretics (Thiazide or Loop)
May enhance potassium depletion.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Increased risk of gastrointestinal ulceration and bleeding.
CYP3A4 inhibitors (e.g., Ketoconazole, Erythromycin)
May decrease methylprednisolone clearance, leading to increased effects.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare. Symptoms may include Cushingoid features, muscle weakness, and hypertension. Management is supportive and symptomatic; gradual withdrawal is necessary after prolonged high-dose therapy.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Methylprednisolone is excreted in breast milk; caution should be exercised when administered to a nursing woman.
