Description
Overview of the medicine
Metrion 400 mg Tablet is a broad-spectrum antibiotic belonging to the fluoroquinolone class. It is used to treat a wide range of bacterial infections by inhibiting bacterial DNA synthesis.
Uses & Indications
Dosage
Adults
400 mg once daily for 7-14 days, depending on the infection severity and type.
Elderly
No specific dose adjustment is typically required unless renal function is impaired. Monitor for adverse effects.
Renal_impairment
Dose adjustment required for creatinine clearance < 50 mL/min. For CrCl 30-49 mL/min: 200 mg every 24 hours. For CrCl < 30 mL/min: 200 mg every 48 hours or 100 mg every 24 hours.
How to Take
Metrion 400 mg Tablet should be taken orally, with or without food. It is recommended to take it at the same time each day to maintain consistent drug levels. Do not crush or chew the tablet.
Mechanism of Action
Metrion inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and ultimately bacterial cell death.
Pharmacokinetics
Onset
Within 1-2 hours for therapeutic effect.
Excretion
Primarily excreted unchanged in the urine, with some fecal excretion.
Half life
Approximately 7-10 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract; bioavailability approximately 90%.
Metabolism
Minimally metabolized in the liver (less than 10% of the dose).
Side Effects
Contraindications
- Hypersensitivity to Metrion or other fluoroquinolones.
- History of tendon disorders related to fluoroquinolone use.
- Epilepsy or other seizure disorders.
Drug Interactions
NSAIDs
Concomitant use may increase the risk of CNS stimulation and convulsive seizures.
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk. Monitor INR closely.
Theophylline
May increase theophylline levels, leading to toxicity. Monitor theophylline levels.
Antacids (containing magnesium, aluminum) / Sucralfate / Iron / Zinc supplements
May reduce absorption of Metrion. Administer Metrion at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be considered. Symptomatic and supportive treatment should be provided. Adequate hydration should be maintained. Hemodialysis or peritoneal dialysis is not effective in removing Metrion.
Pregnancy & Lactation
Pregnancy Category C. Metrion should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk and should be avoided during breastfeeding due to potential risk of arthropathy in the infant.

- Liver function tests (ALT, AST) if liver dysfunction is suspected.
- Complete blood count (CBC) during prolonged therapy.
- INR monitoring if co-administered with warfarin.
Doctor Notes
- Emphasize the importance of completing the full antibiotic course to patients.
