Description
Overview of the medicine
Metronidazole is an antibiotic and antiprotozoal medication used to treat a wide variety of bacterial and parasitic infections. It belongs to the nitroimidazole class of drugs.
Uses & Indications
Dosage
Adults
Dosage depends on the infection and severity. For amoebiasis: 400-800 mg three times daily for 5-10 days. For other infections: 200-400 mg two to three times daily for 5-10 days, or as per physician's instruction. Children's dosage is weight-based, typically 7.5 mg/kg three times daily.
Elderly
Similar to adult dosage, but caution and dose adjustment may be needed in patients with impaired liver or kidney function.
Renal_impairment
No dosage adjustment is generally needed for mild to moderate renal impairment. For severe impairment (CrCl <10 mL/min), dosage may be reduced by 50% for maintenance doses.
How to Take
Take orally with or after food to minimize gastric discomfort. Complete the full course of medication as prescribed by your doctor, even if you feel better.
Mechanism of Action
Metronidazole works by damaging bacterial DNA and inhibiting its synthesis, thereby preventing bacterial growth and replication. It is selectively toxic to anaerobic bacteria and protozoa.

Onset
Within 30 minutes to 1 hour.
Excretion
Mainly excreted in urine (60-80%) and a small amount in feces (6-15%).
Half life
Approximately 6-11 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration.
Metabolism
Primarily metabolized in the liver by hydroxylation, oxidation, and glucuronidation.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives
- •First trimester of pregnancy (relative contraindication)
- •Severe hepatic dysfunction
Drug Interactions
Alcohol
Disulfiram-like reaction (abdominal cramps, nausea, vomiting, headaches, flushing) when consumed with alcohol.
Lithium
May increase plasma lithium levels, leading to lithium toxicity.
Warfarin
May enhance the anticoagulant effect of warfarin and other oral anticoagulants, increasing the risk of bleeding.
Cimetidine
May prolong metronidazole half-life.
Phenytoin/Phenobarbital
May reduce metronidazole plasma levels.
Storage
Store below 30°C in a cool, dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include nausea, vomiting, ataxia, and seizures. Management is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Use with caution during pregnancy, especially in the first trimester. Metronidazole is excreted into breast milk; caution is advised for breastfeeding mothers.
