Metronidazole is a nitroimidazole antibiotic used to treat various bacterial and parasitic infections. It is effective against anaerobic bacteria and certain protozoa.
Uses & Indications
Dosage
Adults
Usually 500 mg intravenously every 8 hours, infused slowly over 30-60 minutes. Dosage varies based on indication and severity.
Elderly
No specific dose adjustment for elderly patients with normal renal and hepatic function, but caution is advised due to potential decreased liver function.
Renal_impairment
Dose adjustment may be necessary in severe renal impairment (CrCl <10 mL/min). A reduction by 50% for patients undergoing hemodialysis is often recommended, administered after dialysis.
How to Take
Administer intravenously by slow infusion over a period of 30-60 minutes. Do not administer as a rapid bolus injection. Always check for compatibility if mixing with other solutions.
Mechanism of Action
Metronidazole is a prodrug. After entering anaerobic cells, it is reduced to active metabolites that interact with DNA, causing strand breakage and inhibition of nucleic acid synthesis, leading to cell death.

Side Effects
Contraindications

Drug Interactions
Alcohol
Disulfiram-like reaction (abdominal cramps, nausea, vomiting, headaches, flushing). Avoid alcohol during and for at least 3 days after treatment.
Lithium
May increase serum lithium levels, leading to lithium toxicity.
Warfarin
Increases anticoagulant effect, leading to increased risk of bleeding. Monitor INR closely.
Cimetidine
May prolong metronidazole half-life and decrease its clearance.
Phenytoin/Phenobarbital
May reduce metronidazole plasma levels.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, seizures, and peripheral neuropathy. Management is supportive and symptomatic; there is no specific antidote.
Pregnancy & Lactation
Avoid during the first trimester of pregnancy; use in later trimesters only if clearly needed. Metronidazole is excreted into breast milk; breastfeeding should be temporarily discontinued during treatment and for 12-24 hours after the last dose.
