Description
Overview of the medicine
Metsina 200 mg Suspension is an antibiotic medication primarily used to treat various bacterial and parasitic infections. It belongs to the nitroimidazole class of antibiotics and works by stopping the growth of certain bacteria and parasites.
Uses & Indications
Dosage
Adults
Typically, 400 mg three times daily for 5-10 days for amoebiasis; 200-400 mg three times daily for 5-7 days for other infections. For suspension, dosage adjusted by volume.
Elderly
No specific dosage adjustment for elderly with normal renal and hepatic function. Use with caution due to potential for reduced hepatic metabolism.
Renal_impairment
Dose reduction may be necessary in severe renal impairment (CrCl <10 mL/min). Administer 50% of the usual dose every 12-24 hours. No adjustment needed for mild to moderate impairment.
How to Take
Metsina Suspension should be taken orally, usually with food or milk to minimize gastrointestinal upset. Shake the bottle well before each use. Use the provided measuring cup or spoon to ensure accurate dosage.
Mechanism of Action
Metronidazole is a nitroimidazole derivative that acts as a prodrug. It is selectively taken up by anaerobic bacteria and sensitive protozoa. Once inside, the nitro group of metronidazole is reduced by electron transport proteins to a highly reactive nitro radical. These radicals damage DNA and other vital biomolecules, leading to cell death.
Pharmacokinetics
Onset
Therapeutic effects usually observed within 24-48 hours, depending on the infection type.
Excretion
Mainly via urine (60-80% as metabolites and unchanged drug), some via feces (6-15%).
Half life
Approximately 8 hours (range 6-12 hours).
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract (about 80% within 1 hour).
Metabolism
Extensively metabolized in the liver, primarily by oxidation, to hydroxyl and other acidic metabolites.
Side Effects
Contraindications
- Hypersensitivity to metronidazole or other nitroimidazole derivatives.
- First trimester of pregnancy (relative contraindication, generally avoided).
- Concurrent use of alcohol or disulfiram.
Drug Interactions
Alcohol
Disulfiram-like reaction (abdominal cramps, nausea, vomiting, headaches, flushing) can occur due to inhibition of alcohol dehydrogenase.
Lithium
May increase serum lithium levels, leading to lithium toxicity. Monitor lithium levels.
Warfarin
May enhance the anticoagulant effect of warfarin and other oral anticoagulants, leading to increased risk of bleeding. Monitor INR closely.
Disulfiram
Concomitant use can lead to psychotic reactions; avoid co-administration.
Phenobarbital/Phenytoin
May decrease metronidazole plasma levels by inducing hepatic enzymes.
Storage
Store below 30°C, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, and mild disorientation. There is no specific antidote. Management involves symptomatic and supportive treatment. Gastric lavage may be considered if ingestion is recent.
Pregnancy & Lactation
Category B for pregnancy, but generally avoided in the first trimester. Use with caution in later trimesters only if clearly needed. Metronidazole is excreted into breast milk; consider temporary discontinuation of breastfeeding or alternative treatment during therapy.
