Description
Overview of the medicine
Telmisartan is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events such as heart attack or stroke in high-risk patients.
Uses & Indications
Dosage
Adults
For hypertension: Initial dose 40 mg once daily. Dosage may be increased to a maximum of 80 mg once daily after 4-8 weeks if needed. For cardiovascular risk reduction: 80 mg once daily.
Elderly
No dose adjustment generally required based on age, but close monitoring for adverse effects is recommended.
Renal_impairment
No dose adjustment is required for patients with mild to moderate renal impairment. For severe impairment, caution and monitoring are advised.
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole with water. It should be taken at approximately the same time each day.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor, which is found in many tissues, including vascular smooth muscle and the adrenal gland. This inhibition leads to vasodilation, reduction in aldosterone secretion, and overall decreased blood pressure.

Onset
Within 3 hours, peak effect achieved within 4-8 weeks of therapy.
Excretion
Primarily excreted unchanged in feces via biliary excretion (>97%). A small amount is excreted in urine.
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Absolute bioavailability is approximately 42% for a 40 mg dose and 58% for a 20 mg dose.
Metabolism
Metabolized by conjugation to form an acylglucuronide, which is pharmacologically inactive.

Contraindications
- Hypersensitivity to telmisartan or any component of the formulation.
- Pregnancy (especially second and third trimesters).
- Lactation.
- Biliary obstructive disorders.
- Severe hepatic impairment.
Drug Interactions
Digoxin
Concomitant use may increase plasma digoxin concentrations.
Lithium
Concurrent use may increase serum lithium concentrations and lead to lithium toxicity.
Diuretics (e.g., Hydrochlorothiazide)
May increase the hypotensive effect, leading to symptomatic hypotension.
NSAIDs (e.g., Ibuprofen, Indomethacin)
May reduce the antihypertensive effect of telmisartan and increase the risk of renal impairment.

Limited data on overdose in humans. The most likely manifestations would be hypotension and tachycardia; bradycardia could also occur. Treatment is symptomatic and supportive. Telmisartan is not removed by hemodialysis.
